In this specialist position with the Westmoreland County Complex Case Division, you will be responsible for overseeing case coordination and follow-up for adults and youth involved in the forensic system. This involves supporting individuals with complex behavioral health, developmental, and social service needs who intersect with the criminal justice system and require multi-system coordination. Our hybrid model provides direct case coordination for people with high-needs forensic cases while also providing short-term follow-up for people with lower-intensity forensic cases. Work includes creating and implementing Case Coordination Plans (CCP) to ensure a structured, organized approach to coordinating forensic cases that involve multiple systems, such as Behavioral Health & Developmental Services (BH/DS), Criminal Justice & Reentry Services, Homelessness & Housing Programs, and Higher Levels of Care. You will also serve as an advocate and liaison by ensuring that individuals meeting Complex Case Division criteria receive appropriate supports, as well as working closely with Complex Case Specialists, legal representatives, and forensic teams to provide an accurate and comprehensive picture of an individual's needs, progress, and required interventions. If you have a deep understanding of forensic and cross-system coordination and the ability to advocate for system wide improvements in forensic behavioral services, we want you on our team!
