Summary: TheDirector, Community Health Outcomes and Analytics is responsible for leading the development, implementation and ongoing management of programs to improve patient outcomes by advancing the scale and spread of evidence-based Social Drivers of Health (SDOH) initiatives across UNC Health. The Director is responsible for overseeing multiple, simultaneous initiatives, and ensure consistency and alignment with the system strategy, Community Health and Engagement strategy and goals of the Clinical Integration unit. Responsible for team culture and team member development. The ideal candidate has experience in Quality or Performance Improvement. Additional experience in Project Management, Social Drivers of Health (SDOH) initiatives, Program Development and Implementation highly preferred. Responsibilities: Health Equity Operations - Liaising with Health Equity leadership to advance Health Equity goals and strategies. Plans and monitors the day-to-day operations of the health equity staff and objectives. Regularly evaluates the efficiency of the work objectives of the health equity initiatives and goals. Revises and/or formulates policies and promotes their implementation. Leading People - Leads people toward meeting the organization's vision, mission, and goals. Provides an inclusive workplace that fosters the development of others, facilitates cooperation and teamwork, and supports constructive resolution of conflicts.
