Job Summary

Job Summary

The Patient Navigator's responsibility is to support the clinical coordination tasks of the Division of Genomic Medicine. Duties include but are not limited to:
* Contacting patients/patient families who are affected by genomic disorders
* Scheduling assessments with various clinicians
* Creating and maintaining patient charts and databases
* Organizing and entering data within the division
* Serving as a non-clinical resource and liaison for patients and their families
* Providing individualized support throughout the hospital stay and/or clinic visits

Minimum Qualifications - For full consideration, applicants are encouraged to upload a license and/or certification if required for the position

  • Bachelor's degree in Genetics/Genomics or a related field
  • Experience with the use of a personal computer and software, including, but not limited to, Word, Excel, and various industry-oriented Electronic Data Capture programs to collect and organize data
  • Ability to maintain confidentiality, exercise discretion, use independent and mature judgment, work independently without supervision, and commit to excellence
  • Skill and ability to recognize and handle difficult/unpleasant situations and/or individuals
  • Serves as the Department's resource person knowledgeable about the Genetics division and its associated programs and personnel
  • Ability to represent the Genetics team members with tact and discretion to patients, families, and others
  • Excellent organizational and planning skills to prioritize and coordinate multiple complex and detailed projects simultaneously, and timely completion of tasks
  • Ability to work efficiently regardless of frequent interruptions and changes in priorities
  • Excellent written and verbal skills to effectively communicate information to health care personnel, patients, and their families in a professional, clear, and factual manner
  • Demonstrate sensitivity and responsiveness to a diverse patient population
  • Demonstrate empathic listening skills

Preferred Qualifications

  • Experience in Pediatrics and Genetics
  • Ability to understand and implement the requirements of providing patient care within a highly regulated and constantly changing environment
  • Ability to understand health care issues and apply these issues to the clinical programs.
  • Strategic planning and evaluation skills; skills to analyze, define, and assess problems, issues, and needs; establish objectives; develop and implement short-term and long-term solutions/plans
  • Ability to anticipate problems/roadblocks and associated consequences, and take appropriate initiative on behalf of program directors
  • Knowledge of and ability to use medical terminology appropriately
  • Personal initiative and ability to accept responsibility for decisions and actions

Department Overview

The UC Davis MIND Institute provides comprehensive evidence-based assessments and intervention recommendations for children with possible neurodevelopmental conditions. The institute is a unique, interdisciplinary research, clinical, and education center committed to deepening scientific understanding of autism and other neurodevelopmental conditions. It is a highly collaborative center, bringing together families, researchers, clinicians, community leaders, and volunteers with the common goal of developing more personalized, equitable, and scientifically proven systems of support and intervention. The institute has major research efforts in autism, fragile X syndrome, chromosome 22q11.2 deletion syndrome, attention-deficit/hyperactivity disorder (ADHD), and Down syndrome.

Department Specific Job Scope

The Patient Navigator will facilitate patient-centered care that is compassionate, appropriate, and effective for the treatment of patients with complex genetic syndromes and disorders. Many of these patients require complex care coordination for imaging and testing studies. Follow-up is of utmost importance to ensure quality of life. Due to the lack of genetic expertise in our community, our patients frequently travel from several hours away, including Oregon and Nevada, to receive care that needs to be patient-centered to be effective. As the Patient Navigator for the genetics program, the position will navigate patients through the health care system to ensure timely screening, diagnosis, treatment, and supportive services. The Patient Navigator will work with other health care professionals to establish and maintain a climate of mutual respect, dignity, diversity, ethical integrity, and trust, and participate in multi-disciplinary teams to provide patient care that is safe, timely, efficient, effective, and equitable. The Patient Navigator will use knowledge of their role and the roles of other health care professionals to appropriately assess and address the needs of patients served to optimize health and wellness.
