Hourly Pay Range:

$28.47 - $42.71 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

What you will do:

The Lead Mental Health Patient Care Navigator plays a pivotal role in guiding and actively participating in the Mental Health Navigation team. As part of this team, your responsibilities include conducting telephonic screenings for patients referred to mental health treatment by their physician or other provider. You will provide patients with access and referrals for a wide range of mental health and substance abuse services, including but not limited to hospital-based treatment and outpatient therapy/psychiatry.

In addition to managing a Navigation caseload, you will provide essential support, training, and mentorship to your team, guiding them to enhance their skills in screenings, documentation, and the effective utilization of community resources. You will oversee the daily queue of Mental Health Navigation orders, identifying process improvements and consistently collecting and reviewing data. Additionally, you will foster relationships with both internal and external healthcare providers, prioritizing relationship maintenance, addressing concerns, and serving as a valuable extension of the entire healthcare team.

What you will need:

  • Education:

Required Bachelor's Degree in a mental health field

Current State of Illinois Mental Health Professional license such as: Social Worker (LSW/LCSW) or Counselor (LPC/LCPC)

  • Certification:

CPI (Crisis Prevention Institute) and CPR (Cardiopulmonary Resuscitation) certifications are offered on-site at no cost and must be completed within the first 90 days of employment.

  • Experience:

Preferred 3 years or more of experience in crisis/emergency services or a related mental health setting

  • Unique or Preferred Skills:

Preferred master's degree or higher in a mental health field
