Project Access Lead Patient Navigator (Bilingual Spanish Required)

Under the supervision of the Program Supervisor, facilitates the delivery of donated specialty care to low income, uninsured and underinsured patients in the New Haven area. Responsibilities include training newly hired Project-Access Patient Navigators, Assisting Program Coordinator with data management system training, supporting program expansion training at new sites, evaluating patients referred to Project Access of New Haven (PA-NH) for enrollment in the program, referring ineligible patients to other sources of medical support, coordinating medical appointments and ancillary services for enrolled patients, coordinating the application process for prescription assistance, reminding patients of appointments, coordinating nonclinical services, and maintaining appropriate documentation of patient contact, referrals, and services provided. The Lead Patient Navigator is seen as the go-to person for navigation needs and is a valued resource between the New Haven Community, YNHH and PA-NH

Responsibilities

  • 1. Provides staff supervision
    • 1.1. Acts as the go-to contact person for navigation team staff for questions and concerns
      1.2. Provides feedback to manager for probationary period, and annual performance reviews
      1.3. Assigns work related tasks to team and monitors productivity and outcomes
      1.4. Leads team meetings including monthly meetings and daily huddles, to discuss daily workload and upcoming work items.
      1.5. Conducts one on one feedback meetings with Patient Navigators.
  • 2. Provides training and department orientation to newly hired PA-NH patient navigators and staff as appropriate
    • 2.1. Assists with interview process for new PA-NH Patient Navigators
      2.2. Responsible for training of newly hired PA-NH Patient Navigator staff in use of Clara Data Management System, navigation process including enrollment and reevaluation process
      2.3. Works collaboratively with Director of Research and Evaluation on patient data management and recommends changes and modifications
      to the system as appropriate
      2.4. Serves as the organizational lead for community resources for PA-NH patient who need community referrals
  • 3. Supports PA-NH program expansion through training at partner sites
    • 3.1. Supports Program Supervisor with writing program curriculum for training at partner sites
      3.2. Conducts community in-services on program as necessary
      3.3. Organizes and conducts community training/workshops on PA-NH organization and related concerns as needed
  • 4. Participates in PA-NH physician awareness and community partnership meetings
    • 4.1. Attends, as appropriate, meetings with hospital and community physicians and community partners
      4.2. Participates in and contributes to weekly staff meetings
      4.3. Attends steering committee or board meetings as appropriate
      4.4. Organizes and conducts community workshops and trainings on program and related topics as needed
  • 5. Screens referred patients for acceptance into the PA-NH program
    • 5.1. Schedules in-office appointments with patients and conducts screenings to determine eligibility for enrollment in PA-NH
      5.2. Meets with patients, reviews documentation provided for acceptance into program, and answers questions of patients
      5.3. Conducts patient baseline survey
      5.4. Enrolls eligible patients in PA-NH program carefully explaining the patient commitment to the program
      5.5. Establishes rapport with patients, family and caregivers
      5.6. Answers questions about the PA-NH program and services
      5.7. Provides referrals to other local agencies as appropriate for services
  • 6. Provides patient navigation services to patients enrolled in PA-NH program
    • 6.1 Schedules/coordinates patient appointments as necessary with Project Access RN Patient Navigator
    • 6.2. Provides patient reminder calls for all appointments
      6.3. Assists patients with referrals for transportation services to appointments as necessary
      6.4. Assists with the coordination of interpreter services as appropriate for patient appointments
      6.5. Provides information on discount prescription services
  • 7. Maintains accurate patient records and processes client records appropriately
    • 7.1. Ensures that all necessary patient information is documented in patient (paper) files
      7.2. Maintains patient records securely
      7.3. Documents all client interactions in electronic database with accurate notes indicating interactions with patients
      7.4. Works collaboratively as a team with other Patient Navigator to ensure that each patient receives comprehensive services provided by PANH
  • 8. Conducts financial reevaluation of patients as appropriate during enrollment in PA-NH program
    • 8.1 Meets with patient face-to-face for reevaluation process every 6 months as appropriate while enrolled in PA-NH
  • 9. Enrolls patients in insurance products as appropriate
    • 9.1. Refers appropriate patients to the state based exchange (Access Health CT) for information on commercial insurance products
      9.2. Participates in training for enrollment in the state based exchange through Access Health CT
      9.3. Enrolls patients into insurance products through the state based exchange (Access Health CT) as appropriate
      9.4. In collaboration with the Program Coordinator, plans and executes the dissemination of information during Access Health CT open enrollment to PA-NH patients
  • 10. Performs other duties as assigned.

Qualifications

EDUCATION

Associates Degree required, Bachelors Degree in psychology, sociology, health and/or human services or related field preferred. Patient navigator certification required.

EXPERIENCE

At least 3 years experience in health care or human service setting. Excellent oral and written communication skills, 2 years relevant experience in community health education and/or outreach position. Bilingual Spanish required

SPECIAL SKILLS

Ability to work independently with support and supervision from Program Supervisor and Nurse Navigator. Candidate must be able to work collaboratively and communicate effectively with others, including patients and families, staff, and community health care providers. Must have experience working with patient level databases (Epic, Clara System) and good Microsoft Office Skills. Candidate must be able to thrive in a moderately paced, complex, health care environment where the Patient Navigator works as a key, valued member of the multidisciplinary team.
