Description Maine Medical CenterProfessional - Clinical Critical Care Clinical Pharmacists are integral members of the multidisciplinary care teams, responsible for providing pharmaceutical expertise in high-acuity settings. With more than 100 ICU beds across 6 specialty areas and a Level I trauma service, MaineHealth Maine Medical Center Portland delivers comprehensive medical and surgical care to a spectrum of critical illnesses. Under the general direction of the Director of Clinical Pharmacy Services, or their designee, the Rotating Shift Critical Care Clinical Pharmacist practices at the top of their license across the adult and pediatric ICU's and the Emergency Department, with flexibility to support broader clinical and operational needs as required. During day shift, this role requires rotation through several of our comprehensive care units, including the MICU, SICU, Trauma ICU, Neurointinsive ICU, CICU, CTICU and emergency department. During night shift coverage, the pharmacist oversees all adult ICU's and the PICU. In addition to order verification, the pharmacist participates in daily interdisciplinary rounds, manages complex pharmacotherapy plans, supports integrated drug distribution services, and contributes to academic research and teaching for students, advance practice providers, and pharmacy and medical residents. This position requires strong clinical knowledge and collaboration. Pharmacists are expected to contribute meaningfully to complex care discussions and respond to adult and pediatric medical emergencies. Responsibilities: - Participate in daily rounds as part of the multidisciplinary team. - Review, verify, and optimize medication therapy for urgent, emergent and critical care patients. - Participate in resuscitation efforts, stroke alerts and other emergency code responses - Prepare and/or mix pharmaceutical products in accordance with USP 797 immediate use compounding standards. - Provide consultative services that foster appropriate evidence-based medication selections - Collaborate with multidisciplinary care team members to monitor, assess, recommend changes to and optimize medication use. - Provide drug information consultation to emergency medicine providers, nurses and other clinicians requesting clinical pharmacy services in the emergency department. - Participate in ongoing quality improvement activities and quality management initiatives. - Provide health literacy-sensitive education to patients and caregivers regarding medication use, disease-state management and prevention strategies. - Assist in precepting students, residents and other learners. - Develop training experiences and competencies in collaboration with other clinical staff - Practice in accordance with state and federal laws, rules and regulations - Perform other related duties as required.
