Job Summary The Unit Clerk assists the nursing and medical staff by performing a variety of service activities and clerical tasks that ensures the efficiency of the patient care unit. Provides services to nurses, physicians, healthcare workers, patients, families, and visitors. Gathers, processes admissions, discharges, and transfers of patients, organizes the unit, maintains unit supplies and equipment, maintains a safe environment, communicates effectively, and utilizes technology. Also advocates for the patient and participates in performance improvement.
Responsibilities:
Greets, welcomes, assists, and directs patient/family, visitors, and members of the healthcare team in a courteous manner while incorporating all customer services initiatives into daily activities (e.g. AIDET, Words that Work, etc.) Maintains and tracks all general and department specific equipment. Collects, cleans, assigns and manages unit staff phones in current call-system. Acts as Fire Marshall while on duty, thereby receiving initial and annual Fire Marshall training. Ensures all essential documents are completed during management of the deceased patient. Accesses electronic health record and assists with the completion of death packet forms. Speaks clearly and in a professional manner within three to four rings and call lights using the approved telephone script (Greeting, name, location, and How may I help you?), and then records and delivers messages accurately and timely to the appropriate persons. Assembles and manages health record (slim chart) from admission to discharge by: assembling, maintaining chart universal order, replenishing current forms, labeling, scanning, copying, disassembling on discharge, and preparing for medical record pickup. Assembles admission /discharge kits with supplies while ensuring all patient documents are correctly labeled. Records all admissions/transfers/discharges and equipment in Unit ADT/Equipment logbook. Notifies the appropriate departments regarding admissions, discharges, and transfers. Enters the appropriate information in the computer system accurately, completely, and timely. Accesses and pages all hospital staff as needed or when requested. Performs daily census reconciliations and enters charges accurately, completely, and timely. Utilizes down-time, processes effectively, efficiently, and accurately when automated systems are unavailable. Prints and sends accurate daily census per protocol when requested. Advocates for patient and family rights and communicates issues and concerns to the Charge Nurse and Unit Director. Initiates and ensures safety of all patient valuables. Makes rounds on all patients (at beginning of shift and as needed) to check: accuracy of daily census, verify correct color armbands (e.g. Fall, MDRO, Allergy, etc.) Schedules patient transportation through current Teletracking System Enrolls all patients up for access to their patient portal and assists them in obtaining an e-mail address if necessary. Assists with obtaining the patients correct home numbers for use of the Patient Call Manager. Participates in shift report from off going clerks and collaborates with Charge Nurse regarding supplies needed, admissions, transfers, discharges and observations. Additional duties as assigned.
Qualifications:
High School Diploma or equivalent required Strong computer skills strongly preferred Experience working in a clerical setting desired American Heart Association BLS preferred
