Description

As a Youth and Family Outreach Case Manager, you will work collaboratively with families to facilitate a team process to give families voice and choice with the outcome of youth with behavioral health issues living in a stable family or community setting. Facilitators work to address concerns, create safety plans, and a build system of support for youth and families to help build resiliency, maintain or reunify families, and/or prevent out of home placements.

This role will handle youth group facilitation and outreach to BIPOC communities! Applicants with experience supporting youth and/or individuals in the BIPOC community are encouraged to apply!

QUALIFICATIONS & REQUIREMENTS

* 3 years of experience in Social Worker or a related field - preferred

* Bachelor's degree in Social Work or related field and/or 3 years of experience, which can include lived experience that is applicable to the job and performance.

* Experience with youth with behavioral health issues.

* Experience working with County systems.

* Experience working with culturally and racially diverse populations in lower socio-economic conditions.

* Thorough knowledge and understanding of professional code of ethics.

* Ability to relate positively with both adults and children.

* Understanding of the nonprofit sector.

* Strong organizational and communication skills with the ability to manage multiple priorities.

* Ability to maintain a good working relationship with all co-workers and the general public. Must use good judgement in recognizing the scope of authority.
