DaVita is looking for an NP Lead. The Lead, Nurse Practitioner ("NP Lead") functions autonomously, yet in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESKD and/or CKD patients.

Job Responsibilities

  • Conduct regular clinical chart audits to ensure high standards for documentation, accurate ICD-10 coding, and closure of care gaps.

  • Perform "shadow shifts" and 1:1 clinical coaching with NPs to refine their bedside demeanor, educate on efficient, and CHE quality.

  • Remain an SME on clinical topics, including ESKD/CKD comorbidities, lab interpretation, and new nephrology guidelines.

  • Serve as the designated backfill for the me the NPM, maintaining operational and administrative oversight whenever I am out of office.

  • Participate in EPM and daily huddle syncs, where necessary, to ensure seamless clinical communication across the market.

  • Handle patient and facility escalations for other pods, providing critical stability and specialized care for pods with current vacancies/time off.

  • Act as a powerful voice for the team, surfacing frontline NP concerns to leadership and advocating for resources or workflow improvements.

  • Facilitate and lead NP-specific huddles, focusing on clinical best practices, morale, and "Village" culture.

  • Monitor high-risk patient lists to proactively guide the team on preventing hospitalizations and ER visits.

  • Mentor and onboard new NP hires, personally guiding them through the transition into the DaVita IKC model and the nuances of working with ESKD/CKD patients.

  • Participate market-level "Deep Dive" case reviews for patients with frequent admissions to identify and address the root causes of instability.

    Perform other duties as assigned/discussed, with Market Leadership.

Qualifications:

  • Current license to practice in the state of practice

  • Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire

  • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification

  • Minimum of two (2) years' experience as a Nurse Practitioner (NP)

  • Preferred experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology

  • Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation of chronic patients required

  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook required

  • Must be willing and able to complete home visits as needed

Preferred Qualifications:

  • Experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management

  • Spanish-speaking preferred in some markets

  • Experience in managing chronic diseases (CHF, diabetes, hypertension, diabetes)

  • Manage pop. Health (hospitalization)

The Salary Range for the role is $133,000.00 - $183,000.00/year.
