Summary
This position is responsible for performing assessments, crisis interventions, individual behavioral health counseling, and psycho-educational/support groups to patients with mental health and/or substance use disorders as needed for inpatients, outpatients, and emergency department patients. They are dedicated to patient and family-centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations. Does this position require Patient Care? Yes Essential Functions -Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues-Formulates biopsychosocial assessment, disposition, and treatment plans. -Provides mandated assessments when abuse is suspected (child, disabled adult, elder) and specialized safety assessments when violence is suspected. Plan discharge from care facility to home or other care facility. -Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. -Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan. Full time, outpatient therapist opportunity embedded within a primary care pediatric clinic. Our LMHC will work as a member of the pediatric primary care department to provide short term therapy to patients with clinical mental health conditions including but not exclusive of anxiety, depression, and other mood related conditions as well as ADHD and behavioral dysregulation. Conducts psychosocial assessments, provides clinical services to pediatric patients that address psychosocial, environmental, age specific and cultural issues in order to maximize emotional, social and physical well-being and the effective/efficient use of health care and community resources. LMHC will be available to Pediatricians for consultations and acute assessments during routine clinic encounters. Experience and training with elementary aged (5-11) children as well as CBT preferred. Works effectively as part of an interdisciplinary health care team and is comfortable referring to and collaborating with interdepartmental specialists and programs, outside agencies, and other relevant community resources.
