Job Description

Elwyn seeks an Activity Coordinator who has full responsibility for the programmatic and operational administration and oversight of their assigned Adult Day Services programs.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provides administrative oversight for all program staff by providing guidance related to proper staff coverage and all programming activities with tasks including but not limited to the coordination of services and supports facilitating full access to the benefits of community living in the most integrated setting of the consumers choosing
  • Creates and fosters a positive workplace culture of support, encouragement, and growth by creating shared vision and clearly communicating that vision by having open and transparent communication with program staff and families of consumers, and by recognizing the accomplishments, training and development needs, and aspirations of program staff whose tasks and activities they coordinate
  • Facilitates development of program modifications in response to development/expansion and program evaluation based upon current regulatory expectations; implements those strategies as deemed necessary and appropriate
  • Provides oversight of the program staff to ensure adherence to regulatory guidelines and maximizing the customer experience through tasks including but not limited to improving the nature and quality of consumer experiences through therapeutic activities emphasizing natural supports and community integration
  • Submits required quarterly reports of participant progress to stakeholders in the Interdisciplinary Team with data including but not limited to metrics demonstrating progress towards the achievement of program goals and objectives as defined in their Individual Program Plan (IPP)
  • Manages and responds to requests in multiple areas, including but not limited to File and Records Management of participant and program records
  • Plans, facilitates, leads, and participates in program / administrative meetings, staff meetings, family meetings and meetings with all other stakeholders as needed providing quarterly and annual reports on the progress of each consumer
  • Educates those they lead on all relevant programmatic issues by conducting program staff meetings
  • Collaborates with Quality Improvement personnel and home administrators in order to ensure compliance with internal and external expectations, policies, procedures, and regulatory guidelines to promote and support the maintenance of a safe working environment
  • Delegates day-to-day operational responsibilities to appropriate staff in accordance with job descriptions and as needed
  • Works collaboratively with regional directors and home administrators to share best practices, promote efficiencies, and transfer understanding of Adult Day Services and Elwyn residential services in supporting the same individual by providing guidance to programming activities with tasks including but not limited to the coordination of services and supports facilitating full access to the benefits of community living in the most integrated setting of the consumers choosing.
  • Provides a setting that encourages a balance of clinical and business priorities and promotes best practices across sites which include but are not limited to compliance with Home and Community-Based Services rules ensuring eligible persons with disabilities have full access to the benefits of community living and are offered services and supports in the most integrated settings of their choosing
  • Communicates and clearly and effectively markets program within and outside of Adult Day Services to maximize clear and consistent exposure in order to increase opportunities to provide service to eligible consumers residing homes supported by Elwyn CA
  • Performs cardiopulmonary resuscitation (CPR) and crisis intervention using agency-trained protocols

EDUCATION & QUALIFICATIONS

  • High school or graduate equivalency diploma; bachelor's degree or higher in a human service field or professional licensure (register nurse, licensed vocation nurse, licensed psychiatric technician) preferred
  • Three (3) years direct work experience with persons with intellectual disabilities
  • Familiarity with disabled, disadvantaged populations preferred
  • Demonstrated working knowledge of Title 17 and Title 22 regulations
  • Demonstrated ability work effectively as part of a team
  • Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, and Outlook; PowerPoint, report-writer experience preferred
  • Non-profit human service experience highly preferred
  • Demonstrated strong attention to detail
  • Demonstrated strong time management and organizational skills
  • Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
  • Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
  • Experience using/knowledge of electronic records preferred
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
  • Must be able to perform CPR and crisis intervention using agency-trained protocols
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
  • Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
