The ideal candidate will possess the following qualifications:

• Bachelor's degree in a human services field preferred.
• Certified Psychiatric Rehabilitation Practitioner (CPRP) certification is a PLUS!
• 1 year experience working in the Behavioral Health field/ population in a group home or a residential facility setting preferred.
• Must have a valid and clean driver's license and clean driving record.

The Psychiatric Rehabilitation Worker actively participates in individual assessments and development of individual service plan and goal setting with clinical liaison and supervisor. They educate the direct care staff on how to provide rehabilitation services that correlate to the goals identified on the service plan and how to appropriately document such services at our 10 Resident Community Residential Rehabilitation facility (CRR) in Colmar, PA.

The Psychiatric Rehabilitation Worker is responsible for:

• Clinical Assessment: Evaluate health concerns and conduct physical health assessments
• Medication Management: Dispense medications and monitor effectiveness and side effects.
• Care Collaboration: Work closely with the program Psychiatrist and interdisciplinary EAC team.
• Documentation: Complete accurate charting and clinical documentation using an EMR system.
• Health Education & Support: Provide health education and support recoveryfocused, holistic care.
• Other duties as assigned, as it relates to the position

Earn $22.29 per hour in this full-time role.
