Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings. Adheres to established clinical session requirements. Provides direct care to patients in the ambulatory setting. Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics. Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model. Performs direct patient care at the outpatient offices in accordance with State licensure and Federal rules. Performs phone triage for patient problems/questions for patients under his/her care, and for other patients as requested by physicians. Continually updates the Endocrinology physicians on the status of certain patients that do require their continued evaluation. Oversees care coordination in the ambulatory care settings, as feasible. Assists the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support, education and encouragement. Performs advanced practice skills under the supervision of the Medical Director. Provides expert consultation for clinical issues promoting increased knowledge and skills for the consultee. Participates in the education, clinical research and mentoring of healthcare providers. Facilitates patient/family education in area of expertise. Annually completes mandatory training as assigned Prescriptive privileges per licensure. BASIC KNOWLEDGE: Masters of Science Degree, Graduate of accredited Nurse Practitioner program. Specialty certification required. Baccalaureate in Nursing Certified in Basic Cardiac Life Support (BLS). EXPERIENCE: Two years related experience as RN in acute care hospital or ambulatory care Prior experience with Endocrinology and Diabetes highly preferred.
