Family and Community Care Coordinator

SUMMARY:

The Family &Community Care Coordinator's role is to outreach to and engage families and community organizations in promoting the behavioral health of diverse populations, particularly those that experience health disparities. Responsibilities will be to identify organizations that provide services that support resilience and recovery, such as health and wellness, housing, education and employment, basic needs, law enforcement, etc. and establish agreements with them that outline protocols for communication, referrals, and care coordination.

The Coordinator will assist Gateway staff as a liaison to an organization to follow through on referrals, service planning, coordinating care, and evaluating progress for individual clients. The Coordinator will also assist staff in engaging and supporting family members' involvement in treatment.

RESPONSIBILITIES:
* Maintains knowledge of available providers and resources in the community to address specialized needs.

* Identifies organizations that support populations that experience health disparities, including black, Latinx, homeless, LGBTQ , identifying ways to effectively engage those populations in behavioral healthcare.

* Outreaches to organizations to explore opportunities for collaborating on client care, establishing agreements and memoranda of understanding that outline protocols for referrals and care coordination.

* Informs staff about available services and supports in the community that will assist in clients' recovery.

* Maintains ongoing communication with community organizations to continue strengthening collaborative relationships.

* Participates in community groups that work to advance the health and wellness of citizens, such as the local Health Equity Zone, the Prevention Coalition, school advisory committee, housing coalition, etc,

* Based on needs identified by clients, family members, and staff, seeks out community resources to meet those needs, helping to make the necessary connections.

* Assists staff in coordinating a client's care across providers.

* Develops educational materials and activities to foster family engagement in the treatment of their family member.

* Works with family members on ways that they can be involved in the client's care in a collaborative manner.

* Leads the Consumer Advisory Council, assessing needs and recommendations to inform quality improvement opportunities the develop supportive community relationships.

* Monitors referral data to outside organizations, analyzing it for trends and opportunities to strengthen relationships.

* Assists in ongoing needs assessment and performance improvement activities.

* Advocates for agency policies and standards for client rights and confidential

* Other duties as assigned

QUALIFICATION REQUIREMENTS:

* Bachelor's degree preferred in a social or human service field such as psychology, public health, behavioral health, education, rehabilitation

* At least 2 years' experience working with individuals who have significant behavioral health disabilities (SED, SMI, SPMI, SUD)

* Caregiver or lived experience in the behavioral health field

* Spanish-speaking preferred

NECESSARY SPECIAL REQUIREMENTS:

* First-hand experience in navigating behavioral health systems and social services.

* Understanding of mental health and substance use disorders

* Knowledge of community resources for clients and families with behavioral health disorders.

* Advocacy and coaching skills.

* Strong verbal communication and social engagement skills /

* Valid driver's license and use of in order to perform the major functions of the job.

* Automobile liability insurance coverage for vehicle being used in accordance with Center requirements.

* Ability to work collaboratively as a team to coordinate care for client with intensive behavioral health needs.

Pay Range:

$24.29-$40.07
