Behavioral Health Navigator

Salary:

  • High School or GED + 5 years' experience: $23.00/hour + every other Friday off paid
  • Bachelor's Degree: $25.00/hour + every other Friday off paid
  • Salary is based on education, experience, and licensure or certification

Schedule: Full-Time | Every other Friday off (paid wellness day) | Monday-Friday 8 am-5 pm

Your Role:

The Behavioral Health Service Navigator will serve as a primary point of contact for clients requiring low-intensity, office-based case management services including but not limited to, walk-in care coordination and support to ensure timely access to resources and follow-up care. This role involves engaging clients to assess needs, referral and linkage and provide ongoing support for both short- and long-term needs. The navigator will maintain a caseload of clients, coordinate care with internal and external resources, and ensure that clients have access to appropriate behavioral health and community resources.

Core Responsibilities:

  • Conduct intake needs assessments and ongoing evaluations to identify client needs, strengths, and goals.
  • Implement, and monitor individualized service plans in collaboration with clients.
  • Maintain an active caseload of clients, ensuring timely follow-up and documentation of services.
  • Provide guidance, support, and advocacy for clients to access community resources, healthcare, and social services.
  • Monitor client progress and adjust service plans as needed to meet evolving needs.
  • Facilitate referrals to internal programs and external community resources.
  • Participate in team meetings, case conferences, and professional development opportunities.
  • Provide short-term or ongoing on-site support to clients based on individualized needs and goals.
  • Conduct proactive follow-up with clients to monitor progress, identify barriers to care, and provide ongoing support.
  • Completion of all documentation and tracking in a timely manner
  • Provide other duties as assigned, required or specifically negotiated with supervisor

Knowledge, Skills, and Abilities:

  • Knowledge of behavioral health systems, community resources, and case management principles.
  • Strong interpersonal and communication skills, including the ability to engage and motivate clients.
  • Ability to develop individualized service plans and track client progress.
  • Proficiency in maintaining accurate client records and documentation.
  • Strong organizational skills and ability to manage a caseload effectively.
  • Ability to work independently while collaborating with a multidisciplinary team.
  • Crisis intervention skills and knowledge of best practices in client-centered behavioral health care.
  • Ability to lift 10 pounds

Qualifications

  • High School Diploma or equivalent plus 5 years of supervised mental health or social service experience
  • Bachelor's degree in social work, psychology, education, or human service field preferred
  • Minimum of 21 years of age
  • A valid driver's license, reliable means of transportation, and proof of automobile insurance.
