Provides case management for patients and families to enhance their psychosocial functioning. Performs duties and responsibilities in a manner consistent with mission, values and Mercy Service Standards.
Position Details:
Education: Master's degree required in Social Work, Counseling or another human service-related field. Licensed Professional Counselor (PLPC) or eligibility for licensure preferred. Preferred Other: Co-worker must be able to communicate orally, in writing and demonstrate proficient computer skills. Co-worker must be able to listen and to understand and explain complex situations.
