Children's Long-Term Support (CLTS)


Children Youth and Family Services - Coggs location


Hourly Range: $25.37- $38.03



The Children's Long-Term Support (CLTS) waiver program is a Medicaid Home and Community Based Waiver program which provides services for children with disabilities. The waiver program allows Wisconsin to use Medicaid funding for community-based support and services to promote inclusion, access, and empowerment. The program is administered by Milwaukee County Department of Health and Human Services, Children, Youth and Family Services for residents of Milwaukee County.


Job Summary: Under the direction of the Supervisor, the main purpose of the Enrollment Specialist is to provide support to the CLTS program and Birth to 3 programs within the Children's Disabilities Unit and Behavioral Health Services Intake team. This position collaborates with the BHS Intake team, Counties, CLTS Team to enroll participants into the CLTS program with a contract agency and internal team.



Job Responsibilities:



  • Create electronic files for participants, maintain waitlist, reports, and tracking. Strategically organize reports for case assignment. Collaborate with the Behavioral Health Services Intake team to review paperwork, Medicaid eligibility, and to determine if a participant is vulnerable.
  • Collaborate with BHS Intake and CLTS teams to ensure submissions and information are accurate. Work with internal and external SSC agencies to enroll and disenroll participants. Transfer participants to or out of Milwaukee County for services.
  • Review data from various sources and develop or manage reports that are to be shared and presented to Milwaukee County Leadership, community partners and WI DHS stakeholders.
  • Enter case notes and participant information into the Program Participant System as well as Eligibility and Enrollment Streamlining. Ensure to comply with State of WI CLTS Waiver Manual requirements and meet internal quality assurance benchmarks.
  • Monitor, report, complete projects, and special assignments by establishing objectives, goals, and outcomes. Assist in the development of policies and procedures.
  • Communicate with participants and their families to assist in providing resources and discuss services with families. Monitor Waiver Medicaid enrollments for waitlist participants. Collaborate with State program, MiLES long-term support team to ensure Medicaid enrollment.
  • Participate in State of WI DHS trainings & meetings. Participate in 1:1 meeting, team meetings, and quarterly Milwaukee County program training. Partner with the state's technical assistance staff to correct participant data and enrollment hurdles.
  • Perform other duties as assigned.



Minimum Qualifications:



  • Resident of the state of Wisconsin
  • Valid Wisconsin driver's license with access to an insured and reliable vehicle; both maintained throughout employment
  • Associate degree or higher from an accredited college or university in the field of Social Work, Psychology, Human Services, or in a closely related field
  • At least one (1) year of experience working with Children with Disabilities, case management, or related experience.
  • Advanced knowledge of all related computer and software applications, such as word processing and spreadsheets.
  • Basic knowledge and understanding of Avatar - Netsmart Systems.



Additional Knowledge, Skills, and Abilities: Advanced analysis skills, problem solving skills, ability to learn Third Party Claims Administration and authorizations; ability to identify other programmatic supports, informal and formal, to support families. HSW Children's Program staff should have competencies which demonstrate they understand child and life span development, dynamics of family relationships, family support, and parenting education (e.g., principles methods and approaches, parent guidance and nurturing principles, child and family health and safety concerns, concepts of diversity in family systems, and relationships among family, school, and community). Skills needed for success in this role include customer service, excellent verbal communication, patience, writing succinctly, critical thinking, and a professional approach. HSWs must have the ability to work collaboratively with other units and organizations within and throughout the county. Proficient in common office suite computer programs; data collection, analysis and in the use of general office equipment.
