As a Case Manager-LPN you are responsible for facilitating resident admissions and discharges, and ensuring a seamless transition throughout their journey with us. You will work with our residents and their families as well as internal/external partners to ensure residents receive the highest quality of care and services.



Responsibilities

    The LPN conducts pre-admission assessment of prospective residents.
  • The LPN develops and ensures continuity of dialogue with hospitals/social workers in community and outlying services regarding availability within community.
  • The LPN will coordinate with social services department on discharge planning.


Qualifications

  • Current state licensure as an LPN, in good standing
  • Previous admission/discharge experience preferred
  • Able to read, write, and communicate in English
