Hospice Graduate LPN Licensed

Works under the supervision of a Registered Nurse and performs skilled nursing services as delegated by the Registered Nurse according to physician's orders. The HSP LPN/LVN, licensed GN is expected to complete core training and skilled competencies within the first 90 days in role.

1. Provides skilled nursing care to patients as directed by the Registered Nurse in accordance with the Plan of Care and care center policies.
2. Demonstrates knowledge of home care and competency in discipline specific patient care skills, required by policy and procedures for the provision of patient care.
3. Assists patients in learning appropriate self-care techniques.
4. Helps to achieve and maintain continuity of patient care by assisting in planning and exchanging information with appropriate staff.
5. Completes documentation timely, accurately, and at the point of care, according to industry standards and follows patient care plans as developed under supervision of the Registered Nurse and according to care center policy.
6. Assists the Physician and Registered Nurse in performing specialized procedures for which competency has been established.
7. Stays informed of industry knowledge and nursing best clinical practice standards.
8. Participates in Interdisciplinary Team (IDT) Conferences.
9. Communicates current health status and needs of patients to the Registered Nurse and other appropriate staff.
10. Performs on-call responsibilities and on-call services to patients/families as assigned.
11. Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
12. Performs other duties as assigned.

• Current, unencumbered license to practice as a Licensed Practical/Vocational Nurse specific to the state(s) the employee is assigned to work/cover for the company. May possess either a single state license in each non-compact state and/or a compact license in their state of residence that is accepted in all Nurse License Compact (NLC) participating states.
• Nurse License Compact (NLC) documents the following: The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state's practice law and regulation. Nurses must legally reside in an NLC state to be eligible to have a multi-state license. See list of Nurse Licensure Compact States (NLC).
• Current CPR certification.
• Less than one (1) year of clinical experience as a Licensed Practical or Vocational Nurse (LPN or LVN).
• Candidate must be approved by VP of Clinical
• Demonstrates knowledge of the appropriate skills for communicating with individuals of all ages and/or cognitive abilities.
