Position Description: The Geriatric Care Manager provides supportive services to clients of SOME's Senior Services Department. Provides geriatric care assessment, case management, care navigation, and resource referral to clients in the form of one-on-one support sessions, family meetings, and group sessions.

This position is located at Kuehner House, which provides permanent housing for seniors 60 and over, and is located on Good Hope Road in SE Washington, DC.

Position Overview: The Geriatric Care Manager provides supportive services to clients within SOME's Senior Services Department. Conducts geriatric assessments and delivers comprehensive case management, care navigation, and resource referrals. Services are provided through individualized support sessions, family meetings, and group facilitation to address the unique needs of older adult clients.

Required: High School Diploma/GED with 3-5 yrs. or Bachelor's degree with 1-2 yrs. in social services or case management experience; previous experience providing services in an interdisciplinary care team.

Preferred: Master's Degree in Social Work with LGSW preferred but not required; 1-3 yrs. experience providing services to seniors and low-income populations; 3 yrs. case management experience; previous experience providing services in an interdisciplinary care team.

Required License/Certification: CPR/First-Aid. LGSW preferred but not required. Must be licensed in DC. Valid Driver's License and access to personal vehicle

Schedule: Monday - Friday, 8:30 am - 5:00 pm (40 hours a week)

Expected Contributions:

Clinical Geriatric Care:



  • Act as a liaison between community partners and SOME Senior Services
  • Collect, interpret, and report data to be used in grant development
  • Conduct routine geriatric care assessments to assess and define barriers and identify treatment goals for building residents
  • Develop person-centered care management plans with clients
  • Provide care coordination through case consultation, care training, and psychoeducation
  • Comply with guidance and direction from Senior Services teams to ensure best practices and uphold standards of care
  • Support with daily operations in Southeast and Northwest locations
  • Partner with other clinical service departments within SOME for improved coordination of care and treatment outcomes
  • Coordinate client care with external providers, i.e., CSA's, EPD Waiver, medical providers, etc.
  • Provide emergency supportive services and crisis intervention to residents. Contact necessary medical personnel and clinical staff, provide follow-up and planning, as needed.
  • Develop, implement, and modify community education curricula to promote wellness and aging-in-community
  • Develop engagement and morale-building initiatives, celebrating individual client and program success
  • Receive and coordinate with new clinical program referrals, confirm eligibility, and conduct intake


Case Management:



  • Provide intensive case management services to up to 60 seniors in the SOME residential housing program at Kuehner House and Karin House
  • Provide incontinence, nutritional, and durable medical equipment and supplies to residents, as needed
  • Provide resource referrals in the form of one-on-one support sessions, family meetings and group sessions
  • Collect and report data in accordance with all funding-related requirements
  • Ensure compliance with the guidelines of funding sources and applicable governing bodies to provide case management
  • Report all issues with supportive services to the resident's assigned agency, including EPD Waiver services
  • Implement care interventions with clients
  • Develop case notes, track services, and ensure timely documentation
  • Submission of COV as needed, coordinating with the legal team, perform rapid reengagement treatment planning
  • Provide daily status updates regarding client care to staff and supervisor
  • Conduct home visits and perform regular environmental assessments
  • Collaborate with SOME property management staff and program managers to provide clinical support for residents with identified housing concerns


Intern:



  • Help support clinical staff in conducting screening and interviews for Intern candidates.
  • Assist clinical staff with training and provide direction and guidance to Interns on department operations and procedures.
  • Supervise and monitor intern progress and engagement. In direct contact with the intern field director.
  • Help clinical staff define work requirements and assign tasks
