Job Description

Minimum Qualifications:

1. Master's degree in Nursing.

2. Advanced Practice Registered Nurse (APRN) license.

3. Basic Life Support (BLS) certification.

4. Three (3) years of nursing experience in Occupational Healthand/orAdultMedicine is preferred.Experience in Urgent CareorEmergencyMedicine would behelpful.

Preferred Qualifications:

1. Specialized certifications appropriate to clinical area.

2. Five (5) years of nursing experience, including supervisory experience.

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

1. Completes admission physical exam accurately, thoroughly, and in a timely manner utilizing appropriate growth and development parameters.

2. Orders appropriate labs and radiological testing based on assessment of needs.

3. Assesses patient/family psychosocial needs, educational needs, learning readiness, and barriers to learning during admission process.

4. Provides patient education for the patient and/or family consistent with assessed needs.

5. Communicates with appropriate members of the health care team regarding any and all patient concerns.

6. Communicates professionally with patient/family, other practitioners, physicians, and co-workers.

7. Sets priorities and modifies patient care in response to changing situations. Demonstrates judgment in consulting MD regarding high-risk patients.

8. Collaborates with other disciplines in planning and coordinating patient care. Allows for optimal patient progress toward wellness and healthy lifestyle.

9. Develops an effective, comprehensive plan of care individualized to the age and needs of each patient using established standards of care, knowledge of disease entities, and human growth and development and emotional, social and spiritual needs and following Department of Public Health standards.

10. Facilitates support groups for preoperative and postoperative patients promoting healthy lifestyle, wellness, using a holistic approach, including cultural, religious, and psychosocial needs of the patient/group.

11. Demonstrates knowledge of medications and their administration based on the age of the patient and clinical condition.

12. Prescribes and complies with Nurse Practitioner protocols and Department of Public Health regulations regarding prescriptive practices.

13. Participates and is prepared for weekly multidisciplinary meetings. Collaborates with other disciplines in planning and coordinating patient care including patient treatments and activities to allow for optimal patient progress toward their weight management success.

14. Participates, to the extent possible, in the development and implementation of community based programs and professional in-services or workshops.

Physical Requirements:

1. Prolonged, extensive, or considerable standing/walking

2. Lifts, positions, pushes and/or transfer patients and equipment.

3. Considerable reaching, stooping, bending, kneeling, crouching.

4. Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc.

5. Regularly exposed to the risk of blood borne diseases and other transmissible infections.

6. Contact with patients under wide variety of circumstances.

7. Subject to varying and unpredictable situations.

8. Handle emergency and crisis situations.

9. May have contact with hazardous materials.

Skills & Abilities:

1. Ability to care for chronically ill patients with a broad range of medical problems.

2. Possess the maturity, self-confidence, and ability to follow complex patient management plans in collaboration with physicians.

3. Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.

4. Ability to provide primary care for a caseload of patients according to practice guidelines and hospital policies, procedures and protocols.

5. Leads by example, Positive thinking, focused on goal.

Job Profile Summary

These roles focus on providing high-quality patient care and promoting wellness. APCs work collaboratively with physicians and other healthcare professionals to diagnose and treat a variety of acute and chronic conditions. In addition, this role focuses on conducting comprehensive patient assessments, ordering and interpreting diagnostic tests, developing and implementing treatment plans for acute and chronic illness, providing education to patients and families about preventive care, treatment options, and self-care techniques, and evaluation of therapeutic response to the treatment plan and with adjustment of interventions developed as needed. A management role that supervises employees focusing on tactical, operational activities within a specified area, with the majority of time spent overseeing area of responsibility, planning, prioritizing and/or directing the responsibilities of employees. Goal achievement is typically accomplished through performance of direct and/or indirect reports.A role that supervises all levels of employees. Responsibilities that typically include: Setting goals and objectives for team members for achievement of operational results, problems faced may be difficult but typically are not complex, and ensures policies, practices and procedures are understood and followed by direct reports, customers and stakeholders.
