About Community Life Community Life is TCB's place-based model that provides stable housing as a platform for residents and neighborhoods to achieve success. TCB communities are places of innovation that address some of the most significant challenges facing low-income populations. We engage residents in creating programs and build strategic partnerships with local stakeholders to maintain stable housing and create pathways to opportunities in six key practice areas: youth development, education, workforce development, health, asset building, and community engagement. Position Description: This role is part-time (maximum 19 hours/week) and temporary. Pay rate: $21/hour Reporting to the Community Life Senior Service Coordinator, the Senior Support Services Aide provides support to residents who have functional disabilities and need assistance with personal care, chores, and/or activities of daily living, in order to remain in their own homes. Vulnerable adults who do not have a willing or capable caregiver available when needed, those at risk of institutional placement, and those at risk of abuse or neglect. The Senior Support Services Aide will ensure resident and community success through close work with TCB property management team and local community partners. The SSSA will maintain a caseload and work one-on-one with residents to assist them in achieving housing stability and improved outcomes in health and wellness. The SSSA is an integral part of the housing and property team and plays a critical role in the overall positive culture of the property for the enjoyment of its residents and the respect of its neighbors.
Housing stabilization is the area of focus that will be delivered through direct support to eligible residents to reduce housekeeping violations, emergency room visits, falls etc. The goal is to have Seniors stable in their housing and ensure they have access to health services that will improve their quality of living. Aides will follow the Congregate Care guidelines outlined by the Department of Aging. Essential Functions:
* Ability to work in a crisis - driven environment to reduce isolation and support Seniors with aging in place.
* Focus on the health of Seniors and those who are most vulnerable to increase self-sufficiency.
* Build positive relationships in the community to build capacity and maintain current partnerships to ensure resources are available.
* Conduct home visits to residents on a weekly, monthly or as-needed basis.
* Attend trainings and continue geriatric education as well as attend meetings with site and leadership teams
* Keep site team and Community Life Director appraised of issues affecting Senior residents
* Document and maintain accurate and up to date records in CL Connect.
* Other duties and responsibilities as requested
* Knowledge and experience with Congregate Care program
o Provide daily services to eligible residents at least 62 years of age and in need of assistance in one or more activities of daily living
o Ensure participants receive meals, weekly housekeeping of each participant's apartment, and limited personal assistance with activities such as bathing, dressing, and laundry
* Maintains friendly, efficient, positive customer service demeanor toward customers, clients, and co-workers
* Maintains all assigned apartments in a clean neat and sanitary manner, to protect safety and health of residents
* Inspects and uses judgment in determining which cleaning techniques outlined to follow
* Demonstrates efficient and safe use of housekeeping equipment and solutions as observed by supervisor
* Diligently employs universal precautions when disposing of trash and bio-hazardous materials.
* Keeps cart properly stocked with equipment and cleaning supplies; ensures all chemicals are accurately labeled, and all equipment is in good working condition
* Promotes effective interpersonal and interdisciplinary relationships, maintains professional conduct at all times
* Communicates concerns to Community Life and Property Management regarding home conditions, work orders, residents health etc.
Knowledge, Skills and Abilities:
* Ability to understand and respect values, attitudes, and beliefs that differ across cultures and to respond appropriately to these differences with residents in planning, implementing, and evaluating programs and services
* Experience working with Seniors to support them aging in place.
* Training in issues that are common for Seniors such as anxiety, dementia, isolation, emotional, social challenges, trauma informed care etc.
* Knowledge of palliative care, mental health disorders, substance related disorders, chronic and co-morbid conditions preferred
* Ability to collect, track and understand data in order to assess programs and services and inform strategies
* Operate in a timely manner, with consistency and a high level of integrity and professionalism; provide excellent customer service to residents and partners
* Excellent communication, organization and writing skills
* Strong advocacy skills Education & Experience:
* Highschool diploma or GED
* 2+ years of overall professional experience working with Seniors required
* Proficiency with MS Excel, PowerPoint, Word, Outlook and other computer programs
* Previous engagement, communication and collaboration with residents of diverse socioeconomic and cultural backgrounds preferred
* Experience working with partners and site team members to implement congregate care initiatives
* Extensive training and knowledge of Geriatrics and the Aging process
* Experience working with data management software is a plus
