Compensation: $38.46/hour- $41.21/hour

Position Summary:

Under the general supervision of the Clinical Nurse Manager/Registered Nurse, and the Community Health Center Administrator and in accordance with established policies and procedures, the Licensed Practical Nurse assists medical providers in rendering patient care during clinic visits by performing routine technical tasks and other duties, as required or assigned.

Essential Duties:



  • Collect and review patient information, including medical history, insurance details, and other pre-treatment documentation, prior to visits with a medical provider.
  • Triage patients and gather relevant health information, including measuring and recording vital signs such as blood pressure, temperature, respiration, and heart rate.
  • Prepare patients for examinations and assist providers as needed; administer prescribed medications, immunizations, and injections in accordance with scope of practice.
  • Provide basic wound care, including cleaning and dressing wounds, and report patient conditions and any changes to the appropriate medical provider.
  • Deliver health education to patients and families, including guidance on medications, nutrition, and overall wellness.
  • Accurately document patient care, observations, and responses in progress notes in accordance with Children's Aid policies and procedures.
  • Monitor and manage medical inventory, including reviewing and ordering vaccines, maintaining vaccine records in the CIR Registry, and tracking medication inventory with regular reporting.
  • Conduct visual examinations for children in foster care in accordance with established protocols.
  • Collaborate with members of the healthcare team to support coordinated, high-quality patient care.
  • Maintain patient confidentiality and comply with all applicable privacy laws and regulations, including HIPAA.
  • Follow universal precautions and safety protocols in alignment with organizational policies and procedures.
  • Communicate effectively with supervisors, including the Registered Nurse, Clinical Nurse Manager, and Health Center leadership, regarding patient care concerns and operational issues.
  • Participate in staff meetings, trainings, quality improvement initiatives, and other organizational activities.
  • Promote a respectful, patient-centered environment by ensuring all patients, families, and colleagues are treated with dignity and care; serve as a patient advocate when appropriate.
  • Support ongoing professional development through required trainings and in-service programs.
  • Perform other related duties as assigned.



Minimum Qualifications:



  • Graduate of an accredited Practical Nurse (LPN) or Vocational Nursing (LVN) program.
  • Current and valid Licensed Practical Nurse (LPN) license in the State of New York.
  • At least two years of clinical nursing experience.
  • Minimum High School Diploma/GED required.

    • Bilingual (Spanish/English) preferred, with the ability to communicate effectively in both languages, including interpretation when needed.

    • Communicates clearly and effectively, both verbally and in writing, with attention to detail and active listening.
    • Manages multiple tasks and priorities in a fast-paced environment while maintaining quality and timeliness.
    • Knowledge of Electronic Medical Records (EMR) systems; experience with EClinicalWorks preferred.
    • Demonstrates organization, collaboration, and service-oriented approach in interactions with patients, families, and colleagues.
