Position Title: LTSS Service Care Manager

Work Location: Cameron County- will be doing in home member visits

Assignment Duration: 6 Months

Work Schedule: M- F 8am - 5pm with 1hr lunch break

Work Arrangement: Field-based with in-home member visits

Position Summary: To complete WLI/Tasks. Conduct Biannual visits and document findings. Coordination with multi-disciplinary teams. Follow up calls to members. Any other projects that may arise.

Qualification & Experience:

Candidate Requirements
Education/Certification	Required: Licensed Practical Nurse or Licensed Vocational Nurse	Preferred: with case management and/or home health experience
Licensure	Required: Licensed Practical Nurse or Licensed Vocational Nurse	Preferred:
Years of experience required: 1-2 years of case management or home health experience Disqualifiers: Must be bilingual Additional qualities to look for: time management, customer service, attention to detail,
