Location:
16th & Burnside Recovery Center

Schedule: On-call shifts can vary by day of the week. Shift durations are generally 4 hours. Must be able to commit to taking on 2 shifts a month.

Compensation: *Hourly compensation is based off professional experience, licensure/certifications/education, and internal equity.

Nurse Practitioner: $65.10-$84.63
Physician Assistant: $65.10-$84.63
Naturopath: $68.35-$88.86
Physician: $111.33-$144.73

MINIMUM QUALIFICATIONS: PHYSICIAN (MD/DO)



  • Board eligibility or certification in Internal Medicine or Family Practice.
  • Licensed to practice as a Physician in Oregon.


MINIMUM QUALIFICATIONS: NURSE PRACTITIONER



  • Graduate of an accredited Nurse Practitioner Program and unrestricted licensure to practice medicine in Oregon or eligibility for such licensure.
  • Appropriate board certification.


MINIMUM QUALIFICATIONS: PHYSICIAN ASSOCIATE



  • Graduate of an accredited Physician Associate Program and unrestricted licensure to practice medicine in Oregon or eligibility for such licensure.
  • Board certification with NCCPA.


MINIMUM QUALIFICATIONS: NATUROPATHIC DOCTOR



  • Graduation from a CNME-accredited, four-year Naturopathic Medical program.
  • Must have completed a CNME-certified primary care residency program.


    • OR Must have at least 3 years' experience in good standing in a primary care site practicing a full spectrum of primary care activities. FQHC clinical experience preferred.


  • Licensed naturopathic physician in the State of Oregon.



ALL APPLICANTS MINIMUM QUALIFICATIONS:



  • Must have current BLS certification.
  • Must have full prescriptive powers in Oregon with a DEA number.
  • Must meet CCC privileging requirements as required by FTCA.
  • Available to work during standard Hooper Center hours.
  • Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit. Must be approved by the Childcare Background Check Unit.
  • Valid driver's license preferred. If required to drive, must pass a DMV background check, and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. If required to drive, must pass an initial driver's training within 60 days of being an approved driver and continued recertification training.
  • Meets current OAR requirements for A&D program staff related to DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act and is able to pass the background check required by the OHA

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Essential Clinical and Compliance Duties:



  • Familiarity with site specific standing orders, workflows, communication standards.
  • Provides admission orders for new patients into residential care, including orders for the administration of medications for opioid use disorder.
  • Take appropriate health histories, perform physical exams, review vitals, labs, other studies, prescribe medications, perform procedures, review outside medical records and other forms of treatment as necessary and appropriate to the standard of care provided in the setting for both acute and chronic health concerns.
  • Formulates treatment plans based on available clinical data and works cooperatively with patients and care team to implement treatment plans.
  • Conducts on-going clinical management of patients including the management of both chronic and acute illnesses.
  • Supports provision and documentation of the appropriate level of care according to Oregon Administrative Rules applicable to the facility.
  • Determines and manages minor and/or acute illnesses and initiates treatment.
  • Conducts initial care for emergency situations such as illness or injury and initiates arrangements for continuing care.
  • Prescribes medication or other forms of treatment as necessary.
  • Keeps appropriate records.
  • Participates in compliance, quality assurance and quality improvement activities as directed.
  • Is available to take calls from RNs even when off-site when working.
  • This position will provide after-hours phone coverage for Residential Substance Use Disorder treatment programs on scheduled rotation with colleagues.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.



Collaborative Care:



  • Support pharmacy-based collaborative drug therapy management programs and provide medical visits to support clients receiving treatment.
  • Coordinate with other CCC programs supporting patients in a continuum of services such as Hepatitis C treatment, withdrawal management, residential treatment for substance use disorders, specialty mental health, and overdose prevention programs.
  • Collaborates with other health care team personnel and case management personnel.



Quality Improvement and Practice-Based Learning:



  • Identify opportunities for improvement in medical workflows and logistics. Participate in review and collection of performance and quality Indicators; review and contribute to continuous improvement opportunities and plans.
  • Participate in and, at times, lead provider education sessions on a variety of clinical topics, including helping train and onboard other medical providers and staff.
  • Participate in compliance, quality assurance, and quality improvement activities as directed.
  • Collaborate with the care team to ensure optimal performance on team-based performance metrics.
  • Perform other duties as assigned, including work at other CCC clinical sites and/or off-site at community events.



SKILLS AND ABILITIES:

  • Ability to consider impacts of oppression, structural racism, and individual bias on client outcomes
  • Must be able to interact with co-workers, patients and staff to instill confidence and facilitate communication.
  • Experience in urgent care and/or primary care practice.
  • Ability to display a non-judgmental attitude.
  • Sufficient manual dexterity and physical ability to perform assigned tasks.
  • Ability to manage time and meet deadlines.
  • Knowledge of de-escalation methods or willingness to be trained in these methods.
