Position Title: Transition Specialist
Work Location: Northeast Ohio (Cleveland, Akron, Canton areas)
Assignment Duration: 6 months (possible extension)
Work Arrangement: Hybrid (field visits ~2x/week + remote documentation) - In Person (field visits to nursing facilities & local SSA offices) Position Summary: This position will be assisting members in northeast Ohio with SSDI applications, which requires in person connections with members in SNFs to gather information via interviewing, obtaining signatures, and also trips to the local SSA office to drop off applications and paperwork. This is a new position for growth to expand cost savings throughout the state, as NEO is our largest membership population. Background & Context: The role supports the Health Equity department. The team is diverse, respectful, supportive, and connected. This position is highly autonomous and responsible for launching the expansion of SOAR across the state. Key Responsibilities:
* Review SNF census, navigate internal systems, gather medical records according to detailed SOP for SOAR SSA.
* Meet with member, interview member and family members to gather pertinent information.
* Communicate with members with language and speech difficulties.
* Navigate SNF environment and social workers for assistance.
* Understand SSA nuances, submit required paperwork and follow up timely according to feedback.
* Organized, detailed, task-oriented work.
* Additional field responsibilities include visiting members in nursing facilities and dropping off documents at local social security admin agencies. Qualification & Experience:
* Required: Bachelor's degree OR equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position.
* 2-4 years of related experience: Social work, nursing, skilled long term care facility, experience with documentation, member facing experience with written and verbal communication skills.
* Ideally, some experience with SOAR applications (not required).
* Preferred: SOAR certification.
* Must be able to obtain SOAR certification within the training period.
* Top 3 must-have hard skills:
- ADL (activities of daily living); EHR clinical knowledge to read and interpret charts.
- Writing reports; completing documentation in detail, independent work and troubleshooting skills.
- Communicating with diverse populations including speech and language difficulties.
Working Conditions & Physical Demands: In-person visits to nursing facilities and SSA offices in Northeast Ohio (~2 trips per week). Mileage reimbursed. Travel not anticipated over 2 hours but depends on location. Daily schedule Mon-Fri 8a-5pm EST (30 min lunch). No OT. Training requires camera on and full attendance.
