Job Overview:

The Geriatric Nurse Practitioner is responsible for supporting and advancing geriatric care initiatives throughout Cooperman Barnabas Medical Center (CBMC), with a primary focus on the ELDER Life Program (HELP) and NICHE (Nurses Improving Care for Healthsystem Elders) initiatives.

The Nurse Practitioner will also collaborate with interdisciplinary teams to provide comprehensive geriatric assessment, consultation, care coordination, and program development aimed at improving outcomes for hospitalized older adults. Responsibilities include geriatric consultative services, interdisciplinary discharge planning, patient and family education, participation in interdisciplinary rounds (IDRs), and quality improvement initiatives related to elder care.

The Nurse Practitioner provides rounding and consultative oversight on nursing units as operational needs evolve.

Qualifications:

  • Master's or Doctoral degree from an accredited Nurse Practitioner Program
  • RN License
  • Current NJ APN license
  • NP Certification
  • CDS and DEA licensure
  • BLS certification
  • ACLS certification
  • Experience in geriatrics, internal medicine, hospital medicine, or transitional care - preferred
  • Experience with interdisciplinary collaboration, quality improvement, and program development - preferred

Essential Functions:

Clinical Functions:

  • Performs comprehensive geriatric assessments and evaluations for hospitalized older adult patients
  • Provides geriatric consultation services to interdisciplinary teams
  • Rounds on patients on designated geriatric-focused units, including Units 41 and 47, and additional units as assigned
  • Participates in daily interdisciplinary rounds (IDRs) to support coordinated patient-centered care planning
  • Identifies patients at risk for delirium, falls, functional decline, polypharmacy complications, and hospital-acquired conditions
  • Develops individualized care plans focused on preserving functional status and promoting safe discharge planning
  • Collaborates with physicians, nursing, rehabilitation services, case management, pharmacy, nutrition, and social work teams

ELDER Life Program (HELP):

  • Oversees development, implementation, and sustainability of the ELDER Life Program initiatives
  • Coordinates evidence-based interventions focused on delirium prevention and preservation of cognitive and functional status in hospitalized elders
  • Supports volunteer, nursing, and interdisciplinary staff education related to elder care best practices
  • Monitors program outcomes and assists with reporting metrics and quality indicators

NICHE Program:

  • Supports development and advancement of NICHE initiatives throughout CBMC
  • Serves as a clinical resource for geriatric nursing excellence and elder-centered care practices
  • Assists with implementation of geriatric-sensitive protocols, workflows, and educational initiatives
  • Collaborates with nursing leadership and interdisciplinary teams to improve care delivery for older adults
