Licensed Vocational Nurse Case Manager The Department of Family Medicine of the Keck School of Medicine of USC provides care for every member of the family, across generations and over lifetimes. We aim to enhance the quality of life for underserved and vulnerable patients while educating future generations of highly skilled, compassionate primary care specialists. Our goal is to combine education, patient advocacy, and evidence-based research to advance family medicine and geriatrics. USC Street Medicine is seeking a Licensed Vocational Nurse Case Manager to support our Enhanced Care Management program. The Licensed Vocational Nurse will work on multidisciplinary Primary Care Team to provide case management support and clinical care to people experiencing unsheltered homelessness in Los Angeles. The ideal candidate must have a commitment to providing high-quality care for complex patients, be dedicated to working and developing a collaborative team model of care and have experience working in case management, as well as working with both physical and behavioral health diagnoses. This is a full-time, in-person position in Los Angeles, CA. There is no hybrid or remote schedule. The schedule is M-F, 8a-4p. Essential job duties include, but are not limited to:
Provides case management support services, guidance and education to Patients to ensure improved health outcomes. Including linkage to pertinent community resources, appointment setting, Provides comprehensive medical record documentation that reflects all aspects of care provided during each encounter, including patient history, care coordination efforts, case management tasks, medication list, allergy list, treatments and/or procedures. Accurately and completely documents data for billing purposes. Demonstrates knowledge and skill necessary to provide individualized interventions and services to promote restoration of health and achievement of patient health goals. Conducts follow-ups with patients via phone calls, encampment visits, and visits to other settings where patients can be found. Provides client care coordination in a manner that is non-discriminatory and non-judgmental Demonstrate an understanding of age-related and cultural needs of individuals served. Accurately obtains and records patient vital signs and medical history. Recognizes changes in the patient's physical and mental condition, takes appropriate action by notifying physician and/or nursing supervisor. Accurate and timely completion of Assessments and Care Plans, as well as periodic assessments of patient progress towards care plan goals. Act as a patient advocate and liaison between the patient and community agencies (i.e., Housing for Health, Department of Human Services, hospitals, etc.) and ensure that patient needs are met with rights maintained; consults and collaborates with community providers to ensure continuity of care. Work cooperatively with external partners and organizations effectively communicating Street Medicine's mission Work closely with, and is a part of the larger internal Street Medicine Multidisciplinary team, including Social Work Department, Registered Nurses, Physician/Physician Assistant/Nurse Practitioner, Community Health Workers and administration team. Promotes security by alerting appropriate personnel for safety, fire and other unsafe situations. Reports any unusual occurrences to supervisor. Demonstrates knowledge of crisis intervention, interviewing techniques, psychosocial issues, counseling principles, and case management documentation processes. Maintains files and records, oversees equipment and office supply inventory. Photocopies, collates, faxes, distributes mail, performs errands. Reads and prioritizes incoming mail. Handles or routes as appropriate.
Required Qualifications:
Valid California Vocational Nursing license Valid BLS for Healthcare Provider Valid IV therapy and blood withdrawal (IV/BW) certification Drivers License
Preferred Qualifications:
1 Year experience with unhoused population. Demonstrated intrapersonal skills and knowledge of patient care and/or needs. Must have keen observation, decision-making, and communication skills. Ability to follow orders and work independently within scope of practice. Experience in a complex medical or academic development setting encouraged. Excellent communication skills - both written and verbal. Comfortable interfacing with highest level of university leadership and university supporters/volunteers. Demonstrated affinity for diplomacy and the ability to cultivate and maintain relationships at all levels. Practices a high level of integrity and honesty in maintaining confidentiality. Ability to multi-task effectively and demonstrate strong organizational skills. Sound judgment and decision making A cooperative and team-oriented attitude Effective facilitator in various settings
Follows established USC and department policies, procedures, objectives, performance improvement, attendance, safety, environmental, and infection control guidelines, includingadherence to theworkplaceCode of Conduct and Compliance Plan. This position is a full time in person position and employee must report to work at the SSB Soto Street Building in Los Angeles, CA when scheduled. Work hours and on-site days may be subject to change depending on business needs. The hourly rate range for this position is$31.11 - $35.00.
