POSITION SUMMARY:

The Ventricular Assist Device (VAD) Coordinator and Advanced Heart Failure Advanced Practice Provider (APP) Supervisor provides specialized care to patients with advanced heart failure and durable mechanical circulatory support. This position combines direct patient care (50%) with program coordination responsibilities (50%) with overarching supervisory responsibilities. The APP reports to the Manager of APPs in collaboration with the Cardiac Surgery Service Line Director of the Cardiovascular Institute to include the surgical and medical director of the VAD program.

The APP works collaboratively with the cardiothoracic surgeons and the advanced HF team, working in a multidisciplinary role to evaluate, treat, and manage complex heart failure patients across the inpatient and outpatient settings, while coordinating care for patients supported with left ventricular assist devices (LVADs).

PRINCIPAL DUTIES AND RESPONSIBILITIES:

VAD Coordination

  • Primarily responsible for planning, direction, and operation of the administrative and clinical aspects of the patient coordination component of VAD program.

  • Support perioperative and longitudinal management of VAD patients

  • Coordinate multidisciplinary care across cardiology, cardiothoracic surgery, and ancillary services

  • Facilitate multidisciplinary team meetings and conferences

  • Provide education and support to patients, caregivers, and healthcare teams

  • Provide comprehensive education to patients, families, and caregivers about VAD management, including device operation, troubleshooting, and lifestyle modifications.

  • Develop and deliver training programs for healthcare staff on VAD protocols and best practices by creating and distributing educational materials/resources to care teams.

  • Participate in quality improvement, regulatory compliance, and program development activities to enhance program efficiency

Advanced Heart Failure Clinical Care

  • Evaluate and manage patient within advanced heart failure inpatient and outpatient setting

  • Participates in advanced heart failure consult service and multidisciplinary rounds

  • Performs independent outpatient office visits managing a panel of advanced HF patients including the following: VAD patients, heart transplant patients, patients listed for heart transplant, acute on chronic HF patients, heart failure hospital discharges

  • Demonstrate the ability to understand heart failure disease processes, interpret laboratory results and diagnostic studies (including but not limited to echocardiograms, cardia catheterizations, cardiac MRIs, and stress testing).

Advanced Heart Failure Supervisor

  • Develop, implement, and maintain professional development and clinical education for all direct reports

  • Support APP participation in continuing education, specialty training, certification, and scholarly or quality improvement initiatives

  • Workflow optimization and ensuring top of license practice

  • Foster a collaborative, inclusive culture that supports patient experience, team engagement, and professional excellence.

QUALIFICATIONS

Master's degree from accredited nurse practitioner or physician assistant program. Minimum 5 years of cardiovascular clinical experience. Current licensure as NP or PA. BLS and ACLS certification

EXPERIENCE:

Experience in cardiac care or critical care is required.

Experience as a VAD Implant coordinator strongly preferred

Excellent written, verbal and interpersonal communication skills.

Experience and skills with electronic health record required.

Knowledge of and compliance with RIDOH, Joint Commission, DNV, HIPPA, ACGME and other regulatory bodies and standards.

Computer skills including Microsoft Office.

SUPERVISORY RESPONSIBILITY:

Supervise 4-8 Full time equivalents (FTEs), including but not limited to Advanced Heart Failure APPs and RNs

Pay Range:

$148,824.00-$186,035.20
