Job Overview:

The ED Outreach Coordinator serves as the primary liaison between the Clara Maass Medical Center (CMMC) Emergency Department and its key external partners - including prehospital Emergency Medical Services (EMS) agencies and the broader community. This role is responsible for enhancing communication, identifying educational opportunities, and strengthening the relationship between the ED and the populations it serves. The ED Outreach Coordinator supports the outreach and quality requirements of Clara Maass's Stroke and STEMI programs, and acts as a central resource for hospital staff, physicians, EMS providers, and community stakeholders regarding emergency care access, education, and system improvement.

The ED Outreach Coordinator maintains a visible presence in the CMMC Emergency Department and in the community to build meaningful, sustained partnerships. This individual will review EMS and ED patient care records to identify trends and opportunities for improvement, engage community organizations to address barriers to care and health literacy, and represent the hospital at local, regional, and state-level forums. This role works with Emergency Services leadership and key stakeholders to advocate for and implement system-wide improvements spanning both prehospital and community care.

Qualifications:

Required:

• High School Graduate or GED
• Non-probationary Driver's license
• BLS certification
• Experience in EMS, emergency nursing, or community health, with at least 1 year in a leadership, supervisory, educator, or outreach role
• Strong knowledge of EMS operations, hospital emergency department processes, and regulatory standards
• Demonstrated experience in community engagement, health education, or public health outreach
• Excellent communication, organizational, and interpersonal skills; comfortable presenting to both clinical and lay audiences
• Experience with data analysis and quality improvement methodologies
• Ability to work collaboratively with multidisciplinary and community-based teams
• Proficiency in Microsoft Office and EMS/health data management systems

Preferred:

• Current New Jersey EMT or Paramedic certification
• Associate's or Bachelor's degree in healthcare, emergency management, or related field
• BLS, ACLS, and PALS instructor certifications

Certifications and Licenses Required:

• High School Graduate or GED
• Non-probationary Driver's license
• BLS certification

Essential Functions:

• Serve as the primary point of contact between the Clara Maass ED and EMS providers, fostering strong, collaborative relationships with local EMS agencies and individual field providers.

• Maintain a visible presence in the ED to enable informal communication, provide real-time education, and build partnerships with EMS and hospital staff.

• Coordinate communication between EMS partners and Clara Maass programs, including Emergency Medicine, Cardiology (STEMI), and Neurology/Stroke.

• Provide guided access for EMS providers to hospital leadership to address and act on identified opportunities for improvement.

Community Outreach & Engagement

• Develop and sustain relationships with community organizations, public health agencies, schools, faith communities, and other local partners to promote emergency preparedness, health literacy, and appropriate use of emergency services.

• Identify and address community-level barriers to care, including health equity considerations, by collaborating with hospital social work, case management, and community benefit teams.

• Plan and execute community-facing events, health fairs, and educational campaigns focused on topics such as when to call 911, stroke and heart attack recognition, and injury prevention.

• Serve as the ED's representative at community meetings, public safety forums, and municipal or neighborhood-level events.

• Collect and incorporate community feedback to inform ED program development and identify unmet health needs.

Education & Training

• Develop, coordinate, and deliver ongoing education and training for EMS personnel, including continuing education, skills assessments, and simulation activities.

• Facilitate EMS-related educational outreach programs, including STEMI and Stroke events.

• Create and deliver community education programming in partnership with hospital departments, public health, and community organizations.

• Collaborate with hospital departments and EMS leadership to identify educational needs and develop targeted programming.

• Serve as an instructor for EMS and community educational offerings.

Quality Improvement & Data Analysis

• Review EMS and ED patient care records to identify trends, quality issues, and opportunities for improvement.

• Lead and participate in case reviews and provide feedback on patient outcomes.

• Monitor real-time patient care transitions between EMS and ED staff to identify and implement process improvements.

• Collect, analyze, and report EMS and community outreach data to support operational decision-making, regulatory compliance, and performance improvement.

• Set and communicate quality metrics and track progress toward system-wide improvement goals.

Program Development

• Support programmatic designations (e.g., Stroke, STEMI) by ensuring EMS integration, community awareness, and compliance.

• Develop outreach strategies that align community engagement efforts with the ED's clinical priorities and population health goals.

• Solicit and incorporate feedback from both EMS providers and community partners to identify and address gaps in the care continuum.

Regulatory & Professional Engagement

• Ensure compliance with all relevant New Jersey and federal EMS regulations and standards.

• Maintain current knowledge of state and national EMS certification requirements.

• Attend and represent Clara Maass at local, regional, and state EMS-related meetings, committees, and conferences as appropriate.

Scheduling Requirements:

• Full-Time, Days
• Monday-Friday with occasional evenings or weekends as needed
