Job Profile Summary This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. In addition, this role focuses on performing the following Home Care Nurse duties: Assesses, plans, implements, and evaluates the care needs of a designated group of patients or provides nursing care and identified clinical support tasks within the scope of practice for achievement of the patient's plan of care as directed. Provides professional nursing care to patients. Requires a nursing license. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. An experienced level role that applies practical knowledge of job area typically obtained through advanced education and work experience. Works independently with general supervision, problems faced are difficult but typically not complex, and may influence others within the job area through explanation of facts, policies and practices. Job Overview Utilizing the nursing process, this position will manage and deliver comprehensive home health services to clients with various medical/surgical diagnoses within their place of residence. Responsible for providing nursing care to patients and education to families in their homes; coordinates activities with other community and Agency staff and carries out special activities on assignment. As a key member of the health care team, this position must interact courteously and effectively with patients and their families as well as with co-workers from all Agency departments, community resources, and with patients' physicians in order to facilitate safe and efficient patient care. Job Description Minimum Qualifications: 1. Massachusetts RN Licensure. 2. Basic Life Support (BLS) certification. 5. One (1) year of acute medical/surgical nursing experience. Preferred Qualifications: 1. Bachelor of Science in Nursing (BSN). 2. Professional certification highly encouraged. 3. Three (3) years of acute medical/surgical experience and/or community health experience. 4. IV and other high-tech nursing experience. 5. Health care and medical/surgical experience. Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned. 1. Assesses and implements physical and psychosocial needs of the patient following established Standards of Nursing Practice and VNA procedures. 2. Assesses and implements patient/family learning needs, style and limitations and makes adjustments for delivery of information. 3. Identifies abuse and/or neglect and identifies reporting mechanism per policy. 4. Demonstrates knowledge of the principles of growth and development over the life span. 5. Possesses communications skills needed to effectively understand patient's reactions or responses to treatment. 6. Involves the patient in understanding and become involved in the course of treatment. 7. Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge. 8. Develops a realistic written plan of care toward measurable goals. 9. Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregivers. 10. Provides ongoing interdisciplinary assessment of the patient. 11. Plans death at home when appropriate with patient/family and other members of the health care team. 12. Completes physicians' orders, levels of care, and OASIS on all patients assigned, in accordance with patient care policies. 13. Maintains and demonstrates a continuous awareness of the health needs and resources of the community. 14. Adapts to new or unusual situations without affecting work performance negatively. Performs other related duties as requested by supervisory personnel. 15. Effectively manages assigned caseload, within the team model of care delivery. 16. Demonstrates an awareness of safety and security issues by responding appropriately to emergency situations and following established procedures and protocols utilizing stabilizing techniques. 17. Provides for reassessment and evaluation of those plans of care at regular intervals. 18. Assists in establishing daily work plan based upon patient/family priorities of service and total area needs. 19. Reconciles medications with patient and physician consistently. 20. Informs patients of next visit date/visit schedule. 21. Continuously assesses/evaluates patient and family progress and goal achievement, keeping Clinical Manager advised and justifying continuation and/or termination of services. 22. Utilizes equipment according to policies and procedures. 23. Educates patients of safe/effective use of equipment provided by vendors. 24. Completes admission paperwork per policy. Contacts manager when/if unable to meet documentation policy. 25. Consistently previews orders for accuracy and sign electronically within 24 hours. 26. Frequently checks and responds to agency voicemail, email and mobile phone. 27. Directs discussion/feedback to immediate supervisor and contributes to Quality Improvement plan for agency. 28. Communicates with all contacts as necessary and uses appropriate communication channels. 29. Provides nursing coverage on a rotating basis for evening, weekends, and Holidays. 30. Participates towards area\Agency goals as appropriate. 31. Reports Compliance concerns to the CEO or Chief Compliance Officer when applicable. 32. Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and demonstrates compliance with Tufts Medicine Care at Home policies and procedures. Physical Requirements: 1. While performing the duties of this job, the employee is required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. 2. The employee must regularly lift and/or move 25 lbs and occasionally lift and/or move up to 100 lbs. 3. The employee in this position frequently communicates internally and externally. 4. Must be able to exchange accurate information via phone, email and/or in person. 5. Specific vision abilities apply including close vision, distance vision and ability to adjust focus. 6. The noise level in the work environment is usually moderate. 7. While performing this job, the employee is exposed to weather conditions prevalent at the time. Skills & Abilities: 1. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served. 2. Excellent interpersonal skills. 3. Ability to function well in very busy situations. 4.
