Be You.

The Duke University School of Nursing (DUSON) Community Health Improvement Partnership Program (D-CHIPP) is seeking a compassionate and skilled Family Nurse Practitioner to join our team. In this impactful role, you will expand access to high-quality primary care by working directly with underserved populations through the TROSA clinic, helping individuals address complex medical needs while supporting recovery and long-term wellness.

In your day-to-day work, you will evaluate and treat patients with acute and chronic conditions, provide preventative and therapeutic care, and coordinate services across a network of community providers. You will play a key role in improving continuity of care, reducing unnecessary emergency department utilization, and ensuring patients receive the support they need to manage their health independently. This position offers the opportunity to perform hands-on clinical procedures, prescribe medications, interpret diagnostics, and deliver evidence-based treatments such as Medication for Opioid Use Disorder (MOUD) and care for chronic Hepatitis C.

You will also serve as a vital liaison between Duke University and TROSA, supporting clinical education opportunities and strengthening community partnerships. Your contributions will directly impact patient outcomes, student learning, and the broader mission of advancing community health.

Minimum Requirements:

  • Completion of an accredited Nurse Practitioner program
  • Current licensure as a Nurse Practitioner in the State of North Carolina

Preferred Qualifications:

  • 2+ years of clinical experience
  • At least 1 year of primary care experience
  • Experience working with underserved or vulnerable populations
  • Familiarity with substance use disorder treatment, including MOUD, and chronic disease management

Other Requirements:

  • Maintain current Basic Life Support (BLS) certification
  • Ability to perform minor in-office procedures (e.g., incision and drainage, wound care, cryotherapy, joint injections, nail excision)
  • Strong collaboration and communication skills to coordinate care across multidisciplinary teams
  • Ability to participate in ongoing continuing education activities to maintain licensure and certification

This position is onsite, with work performed at the TROSA clinic and associated care locations. Availability to work one evening per week in the clinic is preferred.

Be Bold.

Position Responsibilities:

  • Deliver patient-centered care by evaluating, diagnosing, and treating individuals with common acute and chronic conditions in a primary care setting
  • Coordinate comprehensive care by referring patients to specialists, urgent care, or emergency services and facilitating communication between providers
  • Perform clinical procedures such as wound care, ear irrigation, cryotherapy, skin lesion removals, joint injections, and other minor procedures as needed
  • Prescribe and manage treatments including medications, diagnostic testing, medical devices, MOUD, and chronic Hepatitis C therapies
  • Interpret and act on diagnostic data by ordering and reviewing laboratory tests and adjusting care plans accordingly
  • Educate patients on disease management, medication adherence, and self-care to promote long-term health outcomes
  • Assess program eligibility by obtaining detailed medical histories and evaluating patients' ability to safely self-administer medications within the TROSA program
  • Strengthen community partnerships by serving as a liaison between Duke University and TROSA to support clinical education and collaborative initiatives
  • Support clinic operations by ordering supplies, recommending care protocols, and contributing to process improvements that enhance access and reduce unnecessary hospital utilization
  • Provide care coordination and support for clinical volunteers and assist in maintaining efficient clinic workflow
