Hourly Pay Range:

$57.69 - $75.77 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Job Description

Exciting opportunity to join Endeavor Health in the Department of Medical Genetics - High Risk Breast Services. Functions in the role of expert clinician, educator, consultant and coordinator of care. In collaboration with the Physicians and Genetic Counselor's, the APN plans, coordinates, monitors and evaluates the clinical care and services for patients and families by providing clinical support to improve access to care, patient care, and outcomes. The APN serves as a resource and primary contact to assist the patient and family in navigating healthcare and coordination of care/treatment in Personalized Medicine.

  • Obtains detailed family histories and constructs family history (pedigree) documents
  • Assesses and analyzes hereditary and nonhereditary disease risk factors
  • Identifies potential genetic conditions or genetic predisposition to disease (i.e. level of risk)
  • Educates and provides emotional support to individual patients and their families
  • Provides direct patient care for patients at risk for or affected by diseases with a genetic component
  • Facilitates genetic testing and interprets test results and lab reports in some cases
  • Assists with downstream activation of positive results whether germline or the MCED
  • High Risk Breast Follow Up (performs breast examinations, orders related radiology testing)/ROR visits
  • Participates in research initiatives, community/provider engagement projects or outreach
  • Cross Specialty support for studies requiring Personalized Medicine input (e.g. PRS, ROR, guidance for other studies)
  • Primary Care Initiatives (e.g. Population Screening Testing)
  • Expands and manages digital outreach and clinical programming including social media and web content
  • Activation strategies (Organization and 3rd party vendor/collaborations initiated)
  • Virtual Offerings patient, community, and provider facing (e.g. educational forums)
  • Collaborates with Specialty and new hospital systems to integrate genetic services as part of their care

Qualifications:

  • License: Advanced Practice Licensure (APN) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
  • Education: Completion of Master's level program in specialty area
  • Certification: BLS/CPR certification required
  • Experience: Prior APN experience preferred

Special Knowledge and Skills Required:

  • Complete City Of Hope course work within 2 years of employment.
  • High level of analytical ability is required. Must be able to collect, analyze, and interpret data in order to solve problems requiring a professional level of knowledge in a specific discipline and/or design relatively complex clinical or administrative systems and programs.
  • Ability to effectively communicate with all health care team members.
  • Must be comfortable making decisions regarding procedures, plans and schedules related to activities, projects, and assignments.
  • Must be proficient in using PC/laptop, computer software, Excel, Word, Power Point, Progeny, LCD projector, and pediatric scale
