General Summary of Position

MedStar Health is looking for a PRN LMSW/LGSWInpatientCase Manager to join our team at MedStar Washington Hospital Center!

The LMSW/LGSW Inpatient Case Manager servesas a member of the Case Management Team.Facilitates the delivery of quality,cost-effective, patient-centered care from pre-admission through post-dischargetimeframe. Ensures the care is designed to meet individualized patient outcomes. Monitors the care and services delivered to selected patient populations during the acute hospital stay working collaboratively with the multidisciplinary team both internal and external to the organization, promotes effective case management and utilization of resources facilitating the continuum of care, and works to achieve optimal clinical and resource outcomes for the acute, and posthospital phases of care.

Primary Duties:

  • Collaborates with healthcare teams to develop and coordinate comprehensive, patient-centered care and discharge plans.

  • Conducts psychosocial assessments and provides emotional and behavioral support for patients and families.

  • Connects patients with community resources and ensurestimelyreferrals based onidentifiedneeds.

  • Maintainsaccuratedocumentation and communicates effectively with caregivers, case managers, and external agencies.

  • Adheres to regulations and safety standards while activelyparticipatingin quality improvement, education, and outreach efforts.

Qualifications:

  • Master's degree in social workfrom a school accreditedby the Council on Social Work(Required)

  • 1-2years-Experience in social work in a hospital setting (Preferred)

  • 1-2 years- Experience in care/case management (Preferred)

Licenses/ Certifications:

  • LMSW-LicensedMaster SocialWorker- State Licensure in the District of Columbia or the State of Maryland, depending on work locationor

  • LGSW- Licensed Graduate Social Worker in the District of Columbia or the State of Maryland, depending on work location

  • CCM- Certified Case Manager (Preferred)

USD $61,838.00 - USD $111,259.00 /Yr.
