JOB SUMMARY

The Occupational Therapist, Home Health, evaluates, plans, and administers comprehensive occupational therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative occupational therapyas prescribed bya physician.

RESPONSIBILITIES AND SCOPE

• Managesan occupational therapycaseload with efficiency andefficacy

• Completesclinicaldocumentation in the medical record per department guidelines and completes department records for billing and statistics.Performsaccurateassessments and documentation for evaluations, OASIS, discharge, and regular visits.

• Manages resources needed to performOTtreatment plan toaccomplishthe goals set for the patient. Provides,modifies, ordiscontinuesoccupational therapy based upon an ongoing assessment/reassessment of the patient's clinical statusin accordance withphysicianorders

• Selects and performsappropriate OccupationalTherapy (OT) evaluation procedures toidentifyand assess problems. Sets treatment goals and devises a treatment plan. Re-evaluates and adjusts the treatment plan in response to the patient's changing condition and discharge plans.

QUALIFICATIONS

Required

• Bachelor's degree in Occupational Therapyrequired with curriculum approved by the American Occupational Therapy Association.

• Current valid NC license in Occupational Therapy.

• Minimum of two (2) years of work experience in Occupational Therapy. Prior Home Health experience preferred.

• Basic Life Support (BLS)required.

• Prior Home Health experience preferred.
