Join our hospice team serving Maine's coastal communities out of Rockport and make a meaningful impact every day as a Hospice RN Case Manager.You'll help patients and families feel supported, valued, and connected while working alongside a compassionate team dedicated to providing exceptional end-of-life care.

In this role, you'll serve as the primary coordinator of care for hospice patients in their homes, developing individualized care plans and ensuring patients receive the support, services, and resources needed to achieve the highest possible quality of life. You'll work closely with physicians, social workers, home health aides, chaplains, and other members of the interdisciplinary team to coordinate compassionate, patient-centered care.As a Hospice RN Case Manager, you'll have the autonomy to manage your schedule, prioritize patient needs, and build strong relationships with patients and families throughout their hospice journey. You'll combine clinical expertise, critical thinking, and care coordination skills to help patients remain comfortable and supported in the place they call home.

This is a full time, 40 hours per week, Registered Nurse 2 vacancy open to candidates with greater than 1 year of RN work experience. This is an exempt, salaried role.

For a limited time, MaineHealth is offering up to $10,000 Sign-on bonus for all eligible Registered Nurses with 0-2 years of experience and $20,000 for experienced Registered Nurses with greater than 2 years of experience!
o Eligible candidates are hired (offer accepted) into a Full or Part time RN position. Bonus amount prorated for Part time hires, per diem hires are ineligible.
o Current MaineHealth member employees are ineligible; former MaineHealth Members are ineligible until greater than 6 months separation from employment.

Summary:
The MH Registered Nurse II - Hospice Case Manager role provides nursing care and coordinates care for terminally ill patients of all ages in their homes utilizing nursing principles, systems, and resources that affect health status and results in maximum outcome achievement.

MH RN II: This level is competent in their clinical role and has demonstrated the ability to care for most patients assigned. Though not able to care for every patient type within their area of hire, demonstrates how to obtain the appropriate resources to develop required skills and knowledge over time. This level included automatic advancement after 1 year as an RN I - must be in good standing and meets RN I requirements.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Graduate of an accredited School of Nursing required; BSN preferred.
  2. License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required. Current BLS certification required or must obtain within 30 days of start date. Valid Driver's License to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy required
  3. Experience: One year of recent RN experience required. Automatic advancement from RN I level if all onboarding requirements are complete, including the New Grad Residency (where applicable).
