The Patient Family Support Specialist in the Children's Crisis Intervention Center coordinates safe, timely, and person-centered transitions of care for patients presenting with acute behavioral health needs. This role partners closely with child psychiatry, nursing, clinical social work, child life, protective services, and community stakeholders to develop, implement, and document discharge plans that align with identified needs, clinical recommendations, legal/regulatory requirements, and the patients and families preferences. The Patient Family Support Specialist also follows up on access to services post-discharge, and works on projects/task forces as assigned by supervisor or manager to improve quality of care and interdisciplinary collaboration.

Qualifications

EDUCATION

Bachelors Degree in Social Work, Counseling, Psychology, Human Services or a related field.

EXPERIENCE

Knowledge of community behavioral health, crisis services, and social service systems preferred. Experience working with children and families preferred.

LICENSURE

Bachelor degree in social work, counseling, psychology, human services, or a related field.

SPECIAL SKILLS

The Family Support Specialist will have the following special skills: empathy, active listening, critical thinking, de-escalation, organizational and computer skills, data tracking skills, and the ability to effectively communicate with and advocate for patients and families.
