Leads in the development of comprehensive health service plans, participates in the daily organizational and treatment /service planning meetings, and monitors client responses to health coordination and prevention activities. Provide case management services to psychiatrically disabled adults and their families in order to maintain and improve their level of functioning in the community. Orients new clients and their families to CSP (Community Suport and Recovery Services Program) and health home services.

- Bachelors degree preferred in psychology, social work, or other human services related field. AA degree or equivalent education level in the human service field required. One-year experience in community mental health preferred.
