Licensed Vocational Nurse (LVN) - Primary Care

Various Locations: Oakland (East and West) and MobileCare Van (Contra Costa and Alameda)

Under the supervision of the Manager, the Licensed Vocational Nurse (LVN) utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio-economic background, and response to medical regimen and treatment. The LVN is responsible for providing nursing care to patients in the ambulatory setting and serves as an important member of the care team, working closely with the RN and physicians to deliver individualized nursing care based on each patient's physiological and psychosocial developmental needs.

The LVN is responsible for chart abstraction and documentation and consistently demonstrates a professional and courteous demeanor while complying with Medical Center House and Telephone Standards. Clinical responsibilities include medication and vaccine administration, laboratory result review and monitoring, treatments, procedures pertinent to pediatric and adult primary care, and patient education.

The LVN also provides clinical coverage and operational support for the Mobile Health Van and School-Based Health Centers (SBHCs), including direct patient care, community outreach support, regulatory compliance, supply and equipment management, and coordination with operational partners to ensure safe and efficient services.

This position requires flexibility in work location and schedule to support multiple UCSF ambulatory clinics, the Mobile Health Van, and SBHC sites. The LVN utilizes ApeX and other UCSF computerized systems and supports departmental policies, infection control, safety standards, UCSF PRIDE values, and AIDET communication fundamentals.

Key Responsibilities

• Provide nursing care in ambulatory primary care clinics, including:
  • Medication and vaccine administration
  • Laboratory result monitoring
  • Treatments and procedures
  • Patient education
• Perform nursing intake according to practice policies and procedures.
• Assess patient needs through observation and communicate findings to the healthcare team.
• Provide age-appropriate nursing care based on patient developmental needs.
• Perform adult primary care procedures within LVN scope of practice, including:
  • Ear irrigations
  • Suture removal
  • Wound care
  • Assistance with clinical procedures
• Assist patients with limited mobility, wheelchair transfers, and transport.
• Perform chart abstraction and documentation in ApeX.
• Administer and document medications while maintaining current pharmacology knowledge and competency.
• Order vaccines and medications and maintain accurate medication records.
• Support patient flow, triage activities, care coordination, and provider workflows.
• Maintain compliance with UCSF policies, infection control standards, regulatory requirements, and quality activities.
• Participate in required competencies, testing, and quality assurance/quality control activities.
• Deliver customer service through telephone, electronic, written, and in-person communication.
• Identify patient education needs, provide education, and make appropriate referrals.
• Respond to patients with empathy and coordinate care with the healthcare team.
• Prepare patients for diagnostic and therapeutic procedures.
• Maintain continuity of care through effective communication with patients, families, and healthcare team members.

Mobile Health Van Responsibilities

• Deliver direct patient care, including:
  • Immunizations
  • Point-of-care testing
  • Specimen collection
  • Wound care
  • Patient education
• Prepare and restock medical supplies, medications, vaccines, and emergency equipment.
• Ensure vaccine cold-chain integrity and compliance with VFC requirements.
• Coordinate logistics with drivers, Environmental Services (EVS), and clinic leadership.
• Support community outreach events.
• Document mobile patient encounters accurately in ApeX.

School-Based Health Center Responsibilities

• Provide clinical support at SBHC sites within LVN scope of practice.
• Prepare clinical areas and maintain supply readiness.
• Maintain infection control compliance.
• Coordinate with school staff, clinic leadership, and operational partners.
• Document SBHC patient encounters accurately in ApeX.

• 1 - 2 years of experience of LVN
• Basic computer skills with Microsoft Word, Excel, and Outlook.
• Experience using electronic medical record systems.
• Demonstrated ability to provide highly personalized patient care.
• Willingness and ability to work in mobile and school-based clinical environments, including variable schedules, outdoor conditions, and multiple community locations.
• Licensed Vocational Nurse in the State of California.
• Active American Heart Association CPR Certification.

Preferred Qualifications

• Proficiency with ApeX.
• Experience as a Licensed Vocational Nurse in a Primary Care/Family Practice ambulatory setting.
• Familiarity with the principles of a Patient-Centered Medical Home environment.
• Experience in mobile health clinics, school-based health centers, or community outreach programs.
• Experience working in FQHC or HRSA-regulated environments.
