$15,000 sign-on bonus or student loan assistance! The position will cover Queens with coverage for Brooklyn when needed. MJHS is a large not-for-profit health system in the Greater New York area. The Admission Nurse provides patient and caregiver education, evaluates home safety, and confirms eligibility for services. Through strong clinical judgment, excellent communication skills, and a patient centered approach, the Admission Nurse helps ensure a smooth transition into home care and sets the foundation for positive patient outcomes.
* Create the initial comprehensive and interdisciplinary patient centered home health plan of care; and update the home health plan of care in collaboration with the physician or other authorized practitioner (AP). * Associates Diploma in Nursing, required; Baccalaureate Degree, highly preferred. | |
