In this role, you will be a vital anchor for our healthcare team, assessing and monitoring all aspects of our individuals' medical and psychiatric care.

  Collaborative Care:Partner with community healthcare providers, nutrition services, and behavior specialists to deliver high-quality, holistic care.

  Care Coordination:Oversee and coordinate all healthcare appointments, ensuring thorough and timely follow-ups.

  
  
• 
  

  Team Education:Train, empower, and delegate medication administration and nursing tasks to the Direct Support Team in accordance with state licensing and Mosaic policies.

  Medical Record Management:Maintain, review, and audit medical records to monitor health concerns and document the delegation of nursing tasks.

  Primary Hours:Generally day hours.

  On-Call Rotation:Every 3rd week (primarily phone-based for emergencies; in-person response is rare).

  On-Call Support:You are never on an island-you will be backed by a strong support network, including a team of supervisors, QIDPs (Qs), and an RN Supervisor.

Schedule:Generally day hours Monday-Friday some on call responsibilities (mainly via phone)
