Job Summary:
We are seeking an experienced Epic Healthy Planet Analyst with an active Epic Healthy Planet certification to support population health initiatives, value-based care programs, and ongoing Healthy Planet optimization efforts. The ideal candidate will have strong expertise in Epic Healthy Planet build, population health workflows, quality reporting, and analytics, with experience supporting care management and payer-driven initiatives.

Required Qualifications:

  • Active Epic Healthy Planet Certification.
  • Experience configuring and supporting Epic Healthy Planet.
  • Strong understanding of population health workflows and value-based care.
  • Experience with quality measures including HEDIS and eCQMs.
  • Experience with care management documentation and utilization management workflows.
  • Strong analytical, troubleshooting, and communication skills.

Preferred Qualifications:

  • Experience with Compass Rose and advanced risk stratification tools.
  • Experience integrating external data sources such as:
    • Claims data
    • Social Determinants of Health (SDOH)
    • Health Information Exchange (HIE) feeds
  • Experience supporting payer and value-based care initiatives.
  • Knowledge of Epic Clarity and Caboodle reporting environments.

Key Responsibilities:

  • Configure, optimize, and maintain Epic Healthy Planet including:
    • Registries
    • Risk scoring models
    • Care gap logic
    • Patient attribution frameworks
  • Analyze population health workflows and translate operational requirements into system enhancements.
  • Develop and maintain:
    • Registries
    • Reporting Workbench reports
    • Dashboards
    • Population health analytics
  • Support quality improvement initiatives including HEDIS and Medicare Stars programs.
  • Design and maintain care management tools including:
    • Care plans
    • Protocols
    • Assessment tools
    • Outreach workflows
  • Support payer and value-based contracts through accurate attribution, measure calculation, and data capture.
  • Troubleshoot data integrity issues related to registries, reporting, and population health metrics.
  • Collaborate with analytics teams to validate data movement between Epic, Clarity, Caboodle, and downstream reporting platforms.
  • Lead optimization initiatives focused on workflow improvements, automation, and reporting enhancements.
  • Participate in governance and prioritization of Population Health enhancement requests.
  • Support testing, validation, upgrades, and production deployments.
  • Provide application support for care managers, quality teams, and operational stakeholders.
  • Assist with implementation of new programs, including registry design, workflow alignment, and measure mapping.
  • Maintain documentation for build standards, workflows, and reporting logic.

This role is ideal for an Epic Healthy Planet professional with strong population health expertise who can support large-scale value-based care initiatives while optimizing Healthy Planet functionality, reporting, and care management workflows.

Schedule:

  • 5 x 8 schedule.
  • Approximately 08:00 AM - 5:00 PM (start/end times may shift earlier or later).
  • 40 hours per week.
