Overview

The Home Infusion and Specialty Pharmacist performs, coordinates, and supervises pharmacy services including preparation and dispensing of drugs, supplies and equipment, preparation of sterile products as they pertain to providing safe and effective therapies in the home and alternative site settings. Provides clinical assessments, education, care planning and monitoring of responses to therapies and therapeutic outcomes. Hours may vary, and include on-call, depending upon census and program need. Main software used: CareTend, NewLeaf, EPIC, TherigySTM, and Abacus.
In the absence of the manager/leader, pharmacists on duty are authorized to assume the duties of the manager/leader to maintain workflow and patient care. These include all of the duties of the Pharmacist-in-Charge (PIC), except those duties restricted to the PIC by local, State, and Federal Law.
Qualifications

Responsibilities

Communicates the results of therapeutic drug monitoring activities to the physician, nurses, and other appropriate persons.
Completes documentation as per policy and procedure in order to maintain a thorough and accurate clinical record for each patient.
Conducts initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs
Formulates a goal directed plan of care, which is prioritized and based on determined patient problems and patient outcomes.
Intervenes as appropriate to improve prescribing, administration and monitoring practices in the facility as related to medications.
Maintains pharmacy records in accordance with applicable laws and regulations.
Performs miscellaneous billing functions including patient charging through manual entry into the computer and generation of billing forms. This includes identifing payor and obtaining auths prior to the delivery of medication or supplies.
Programs infusion control devices as indicated and has ability to teach pateints/caregivers and staff how to use devices.
Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
Supervises, collects, and analyses PI activities pertaining to specific pharmacy functions
Supervises, coordinates and/or participates in the preparation of sterile admixtures and dispenses all pharmaceutical and related supplies and equipment in accordance with federal, state and local laws and standards of practice which govern the progressio
Pharmacist will supervise pharmacy operations and personnel.
Full time pharmacists are required to be on-call as a part of a rotation.

Compensation and Benefits

The compensation range for this position is $52.06per hour - $89.54per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
