About the Role Optum Care at Home is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal, in-home care for high-risk health plan members. Providers work within an interdisciplinary care team to manage complex medical conditions, improve health outcomes, and help patients remain safely in their homes.

Key Responsibilities

  • Conduct comprehensive in-home assessments, including physical exams, medication reviews, and screenings
  • Manage acute illnesses and chronic disease exacerbations
  • Support members during short-term rehabilitation episodes
  • Ensure accurate documentation and ICD-10 coding
  • Collaborate with interdisciplinary care teams
  • Conduct advanced illness and advance care planning discussions
  • Educate patients and caregivers
  • Travel extensively throughout Washington to provide home-based care

Required Qualifications

  • NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • PA: NCCPA certification and active state licensure
  • Active, unrestricted license (or ability to obtain)
  • Active DEA license/prescriptive authority (or ability to obtain)
  • Valid driver's license and reliable transportation
  • Ability to obtain a collaborative practice agreement if required
  • Ability to perform physical assessments in home-based settings

Preferred Qualifications

  • 1+ years of APC experience
  • Experience in geriatrics, home health, senior living, or long-term care
  • Experience caring for medically complex patients
  • Knowledge of chronic illness, geriatrics, advanced illness, and end-of-life discussions
  • EMR and technology proficiency
