About the Role Optum Care at Home is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal, in-home care for high-risk health plan members. Providers work within an interdisciplinary care team to manage complex medical conditions, improve health outcomes, and help patients remain safely in their homes. Key Responsibilities
- Conduct comprehensive in-home assessments, including physical exams, medication reviews, and screenings
- Manage acute illnesses and chronic disease exacerbations
- Support members during short-term rehabilitation episodes
- Ensure accurate documentation and ICD-10 coding
- Collaborate with interdisciplinary care teams
- Conduct advanced illness and advance care planning discussions
- Educate patients and caregivers
- Travel extensively throughout Washington to provide home-based care
Required Qualifications
- NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- PA: NCCPA certification and active state licensure
- Active, unrestricted license (or ability to obtain)
- Active DEA license/prescriptive authority (or ability to obtain)
- Valid driver's license and reliable transportation
- Ability to obtain a collaborative practice agreement if required
- Ability to perform physical assessments in home-based settings
Preferred Qualifications
- 1+ years of APC experience
- Experience in geriatrics, home health, senior living, or long-term care
- Experience caring for medically complex patients
- Knowledge of chronic illness, geriatrics, advanced illness, and end-of-life discussions
- EMR and technology proficiency
