About the Role Optum HouseCalls is seeking Nurse Practitioners and Physician Assistants to provide annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based position focused on preventive care, patient education, care gap closure, and collaboration with primary care providers.

Key Responsibilities

  • Conduct comprehensive in-home health assessments, including medical history review, medication reconciliation, physical exams, and screenings
  • Identify and document diagnoses to support care management and treatment planning
  • Communicate findings and recommendations to primary care providers
  • Educate members on disease management, medications, and overall health
  • Address social determinants of health and connect members with community resources
  • Recognize and appropriately respond to urgent or emergent situations

Required Qualifications

  • NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • PA: NCCPA certification and state licensure
  • Active, unrestricted license (or ability to obtain by start date)
  • Valid driver's license and reliable transportation
  • Ability to perform assessments in a variety of home environments

Preferred Qualifications

  • 1+ years of clinical experience, preferably in family practice, geriatrics, home health, or primary care
  • Experience working with Medicare or geriatric populations
  • Proficiency with EMR systems and technology
  • Strong patient communication and education skills
  • Fluency in a language other than English

Compensation for this specialty generally ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
