As a member of our Senior Community Care (SCC) team, you'll provide care to individuals living in nursing homes, assisted living communities, and senior housing settings. The Senior Community Care (SCC) program focuses on coordinating and delivering comprehensive clinical services to some of our highest-risk health plan members. As an Advanced Practice Clinician (APC), you'll have the opportunity to make a significant impact by providing individualized care, partnering with interdisciplinary teams, and supporting patients through complex health needs. Primary Responsibilities
  • Complete comprehensive patient assessments, including:
    • Medical history review
    • Medication reconciliation
    • Physical examinations and vital signs
    • Evidence-based screenings and point-of-care testing, as appropriate
  • Support members during short-term sub-acute rehabilitation stays
  • Maintain accurate and thorough clinical documentation, including ICD-10 coding
  • Collaborate closely with interdisciplinary care teams to coordinate patient care
  • Lead advanced illness and goals-of-care discussions
  • Educate and counsel patients and caregivers to support informed health care decisions
