Case Manager - Part-time - (Bachelors in Social Work) - Behavioral Health - Dubuque, IA

Horizon Health Corporation is seeking a Case Manager (Part-time) for our behavioral health program at UnityPoint Health-Finley Hospital in Dubuque, IA. The Case Manager is responsible for coordinating services, developing care plans, and advocating for clients to ensure they receive appropriate resources and support. This role works collaboratively with clients, families, healthcare providers, and community agencies to assess needs, monitor progress, and promote positive outcomes. A Bachelor of Social Work (BSW) is required, along with strong communication, organization, and problem-solving skills.

Responsibilities:

  • Conduct client assessments to identify needs, risks, strengths, and goals
  • Develop, implement, and update individualized service or care plans
  • Coordinate referrals to community resources, behavioral health services, housing, financial assistance, and other supports
  • Participates as an active member of the interdisciplinary and treatment teams. Attend Unit and Hospital meetings as directed
  • Monitor client progress and follow up to ensure services are effective and timely
  • Advocate for clients and help remove barriers to care or services
  • Maintain accurate, timely documentation in accordance with agency and regulatory standards. Ensure compliance with confidentiality, safety, and reporting requirements
  • Educate clients and families on available services, benefits, and support systems
  • Conduct discharge planning, transition planning, or ongoing service coordination as needed
  • Performs other duties as assigned/required by this position

Qualifications

  • Bachelor's degree required
  • 0-1 years preferred
  • Valid Driver's License and/or ability to commute to work required
  • Strong knowledge of case management principles, community resources, and social service systems
  • Familiarity with crisis intervention, discharge planning, or care coordination
  • Completes and maintains all required state/regulatory, Hospital and Horizon competencies and training/education expectations on a timely basis
  • Knowledgeable of hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental and infection control standards
  • Enhances professional growth and development through participation in educational programs, in-service meetings and workshops. Keeps up to date on professional literature
  • Fosters sense of teamwork and collaboration within the department and Hospital
  • Ability to manage stress appropriately and channels concerns appropriately
  • Licensure eligibility or certification in social work, depending on state or employer requirements
