The pediatric ambulatory RN is an integral member of the patient's healthcare team and is responsible for performing outpatient care for patients with a specialty diagnoses. The pediatric ambulatory RN is responsible for coordinating and managing care for specialty clinic patients inclusive of newly diagnosed, established, high risk, and noncompliant patients in the clinic setting; is up-to-date on current ambulatory therapies and research; promotes self-management skills and provides collaborative and comprehensive care that optimizes the patients management of their illness; practice is in compliance with hospital policy, procedures and protocols and in accordance with the providers plan of care; acts and serves as a patient and family advocate; establishes trust and builds therapeutically effective relationships with the patients and families.
POSITION SUMMARY: The registered nurse, adheres to the rules and regulations of the Massachusetts Board of Registration in Nursing. is primarily responsible for professional performance and direct patient care. The role encompasses nursing assessment, diagnosis, planning, intervention, evaluation, and coordination of care for patients and their families across the health care continuum. The registered nurse is accountable for clinical, educational, quality and fiscal patient care outcomes using the model of care designed by the nursing department and in accordance with established agency policies, procedures, protocols, guidelines and standards of practice. Demonstrates strong clinical judgment and expertise in the assessment, care coordination, and management of children and adolescents with developmental, behavioral, and neurodevelopmental conditions, including autism spectrum disorder, ADHD, intellectual and developmental disabilities, developmental delays, learning disorders, behavioral health conditions, feeding and sleep disorders, and related genetic syndromes with knowledge of myriad structural and social drivers of care including trauma, poverty and multilingual homes. Demonstrates knowledge and experience navigating special education systems and school districts across the Commonwealth, including the IEP and 504 process, and partners with families to access appropriate educational supports and services. Collaborates with families, providers, schools, behavioral health agencies, and community organizations to coordinate evaluations, therapies, specialty care, and resources that promote optimal patient outcomes. Utilizes standardized developmental and behavioral screening tools, including autism-specific screeners, to identify concerns, facilitate appropriate referrals, and support timely intervention. Works collaboratively within an interdisciplinary team to ensure continuity of care and support successful transitions across the continuum of services. Compensation Range: $38.05- $75.33This range offers an estimate based on the minimum job qualifications.
