Reliant Medical Group, an Optum company, is seeking a BC/BE Geriatric Medicine Physician or an Internal Medicine Physician with a strong interest in Geriatrics to join our expanding post-acute and long-term care team serving a skilled nursing facility in Worcester, Massachusetts.

This is an exceptional opportunity for a physician who is passionate about caring for older adults and enjoys building meaningful relationships with patients, families, and interdisciplinary care teams. Providers will deliver comprehensive medical care in Skilled Nursing Facilities, helping to improve quality of life and health outcomes for a complex geriatric population.

Responsibilities
  • Provide comprehensive medical care to residents in Skilled Nursing Facilities
  • Manage acute and chronic medical conditions in older adults
  • Conduct admissions, follow-up visits, and discharge planning
  • Collaborate with facility staff, specialists, patients, and families to coordinate care
  • Support quality improvement initiatives and reduce avoidable hospital readmissions
  • Participate in interdisciplinary care planning and team meetings
Qualifications
  • MD or DO
  • Board Certified or Board Eligible in Geriatric Medicine, Internal Medicine, or Family Medicine
  • Strong interest in Geriatric Medicine required
  • Eligible for Massachusetts medical licensure
  • Excellent communication and team-based care skills
  • Experience in Skilled Nursing Facility, post-acute, long-term care, or geriatric settings preferred
