Summary:
Presbyterian Heart Group / Pediatric Cardiology is seeking a dedicated and highly collaborative Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing Pediatric Interventional Cardiology Program. This role provides comprehensive care for infants, children, adolescents, and young adults with congenital and acquired heart disease across the full continuum of care.
The APP will partner closely with pediatric interventional cardiologists and multidisciplinary care teams to support cardiac catheterization procedures, outpatient clinic management, inpatient consultations, and longitudinal patient follow-up. Responsibilities include pre- and post-procedure evaluations, care coordination, patient and family education, procedural preparation and recovery management, and collaboration with NICU, PICU, pediatric specialty, and community providers.

Job Description: INTERVENTIONAL CARDIOLOGY / CATH LAB RESPONSIBILITIES
- Participate in pre-procedure evaluation and preparation of patients undergoing diagnostic and interventional cardiac catheterization procedures.
- Coordinate procedural scheduling, laboratory testing, imaging studies, and insurance authorizations.
- Assist with patient and family education regarding procedural indications, risks, benefits, alternatives, and recovery expectations.
- Support daily cathlab operations and communicate with multidisciplinary procedural teams.
- Perform comprehensive pre-procedure history and physical examinations.
- Evaluate and manage patients before, during, and after cardiac catheterization procedures.
- Collaborate with anesthesia and nursing teams regarding peri-procedural planning.
- Participate in post-procedure recovery assessments and discharge readiness evaluations.
- Monitor procedural outcomes and contribute to quality improvement and registry initiatives.
- Support outreach and referral development activities that expand access to pediatric interventional cardiology services.
PRE-PROCEDURE CARE
Pre-Procedure Visits & Evaluation
- Conduct comprehensive pre-catheterization clinic visits and pre-procedure assessments.
- Review diagnostic studies including echocardiograms, ECGs, CT, MRI, and laboratory results.
- Identify clinical concerns and optimize patients prior to procedures.
- Develop individualized care plans in partnership with the interventional cardiologist.
- Educate patients and families regarding procedural expectations, medications, fasting requirements, and recovery instructions.
- Coordinate multidisciplinary consultations when clinically indicated.
- Facilitate pre-operative clearances and communicate findings to procedural teams.
POST-PROCEDURE CARE
Post-Procedure Visits & Follow-Up
- Conduct post-procedural evaluations and scheduled follow-up visits.
- Manage vascular access site monitoring, wound assessment, and recovery concerns.
- Review procedural findings with patients and families and reinforce discharge instructions.
- Coordinate ongoing diagnostic testing and specialty referrals following procedures.
- Respond promptly to post-procedure patient questions, concerns, and urgent issues.
- Ensure appropriate transition of care between inpatient and outpatient settings.
- Document procedural outcomes and track follow-up compliance.
OUTPATIENT CLINIC RESPONSIBILITIES
- Conduct independent and collaborative APP clinic visits under physician co-management.
- Evaluate new and established patients with congenital and structural heart disease.
- Manage longitudinal follow-up for patients undergoing catheter-based interventions.
- Review and communicate diagnostic study results to patients and families.
- Adjust medications and treatment plans within APP scope of practice.
- Provide comprehensive patient and family education regarding diagnosis, treatment, and follow-up.
- Coordinate transition of care for adolescent and young adult congenital heart disease patients.
- Improve clinic capacity and patient access through collaborative physician-APP care models.
CARE COORDINATION RESPONSIBILITIES
- Serve as a key care coordinator for pediatric cardiovascular patients across inpatient and outpatient settings.
- Collaborate with pediatricians, neonatologists, intensivists, cardiac surgeons, nurses, therapists, and community providers.
- Coordinate care for patients within the NICU, PICU, and pediatric inpatient units.
- Facilitate referrals, diagnostic testing, imaging, and specialty consultations.
- Assist families in navigating complex cardiovascular treatment plans and resources.
- Ensure continuity of care throughout the procedural episode and longitudinal follow-up process.
- Participate in multidisciplinary case conferences and care planning meetings.
- Collaborate with referring providers to support appropriate follow-up and shared care management.
INPATIENT RESPONSIBILITIES
- Participate in inpatient consult triage and assessment for pediatric cardiology patients.
- Evaluate patients admitted for catheterization procedures and congenital heart disease management.
- Conduct inpatient rounds in collaboration with pediatric cardiology physicians.
- Assist with discharge planning, care transitions, and follow-up coordination.
- Facilitate communication between inpatient teams and outpatient providers.
Additional Job Description:
- Master's or Doctoral degree as a Nurse Practitioner or Physician Assistant from an accredited program.
- Current state licensure as an Advanced Practice Provider in New Mexico (or eligibility to obtain).
- Pediatric clinical experience or pediatric cardiovascular experience.
- DEA registration and prescriptive authority as applicable by licensure.
- Current BLS certification.
